2018-03-15 | Production Wells | No Violations | On this day, I found the well equipped with annular pressure monitoring devices. The pressure gauges are labeled with surface pressure reading 250 pounds per square inch, and the intermediate gauge reading 0 pounds per square inch. There are (6) total wellheads, all in production, with concrete barriers surrounding them, and an electronic shutdown button at each wellhead. The facility is gated with proper signage, and an electronic shutdown button on the left-side, prior to the pad. The back right-side of the pad is equipped with (4) 400 barrel steel tanks and (1) 50 barrel steel tank all within containment, and an electronic shutdown button is present. There are (6) gas processing units along the right-side of the pad with electronic shutdown buttons on each, and a midstream facility on the right, at the entrance.
2017-03-06 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection accompanied by ODNR Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Hybrid Services completion rig # 102 is on location drilling out the completion plugs on the Roger # 4-A well. The rig is situated on polymer containment with the well pad perimeter earthen berm serving as the primary containment with the pad drain valves closed. There is 6 total wells on the pad that have been drilled and stimulated that will now be completed and the initial flow back operations commenced.
2017-06-27 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the area affected by the well site construction has vegetation established and erosion control in place and is in compliance with ORC 1509.072. Addressing the well site. There is 6 producing wells on the pad all in line and selling product with tubing well heads installed on each well and the production valves in the open position. The wells are selling into gas processing units (GPU) with (6) units present. Also on the well site is the production fluid storage facility consisting of (4) steel production tanks all located inside a polymer lined steel containment dike area. The company has installed electrically controlled emergency shut down devices at each well head and also at the production facility and the well pad entrance. Located along the well site entrance lease road is a mid stream operation owned by Rice Energy in which all the wells on the pad sell into. Signage is posted at the well pad entrance and at each well head.
2016-04-04 | Drill / Deepen / Reopen | No Violations | On site status check inspection on the current well drilling process found H&P Driling rig # 83 on location running in the hole with the drill string to begin drilling the 12.250 2nd intermediate well bore section. The 13.375 was set on 04/03/2016 and cemented back to surface. The Blow Out Preventer test was conducted by Greens Energy Service and reviewed with the company is operating on a 31 day BOP test. The well head is a Cactus well head with a 5,000 psi c section -10,000 psi B section and 15,000 psi A section rated working pressure.
2016-11-07 | Preliminary Restoration | No Violations | Preliminary restoration status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control in place. Also noted was a service rig owned by Hybrid Drilling was on location rigging up to begin the well bore toe prep operations in anticipation of the well stimulation process with the following well head completion equipment in place-A cactus well head and completion stack with a Red Dirt Services Master control valve. The rig and support equipment is placed on polymer containment. Stingray Flow back services is also on location.
2016-03-15 | Casing String Witnessed | No Violations | The said well was spudded on 03/15/2016 with a 110 x 36 conductor borehole being drilled by Butches Rat Hole Service and -110 of 26 .375 wall-weld connection conductor pipe was set and cemented by Stingray Cement Service using 490 sacks of Class A cement with 2% calcium and .125 lb/sk lcm additives. Circulation was established prior to cementing and maintained throughout the job with cement displaced to 100 with 65 barrels of fresh water and cement returns witnessed to surface inside the cellar.
2016-04-01 | Casing String Witnessed | No Violations | On site inspection to witness the surface casing cement job found Nomac Drilling rig # 83 is now on location and has drilled a 24 x 399 surface borehole and ran 394 of 18.625 J-55 Buttress thread casing with a float shoe and collar ran and proper centralizer placement demonstrated. Stingray Cement Service is on location to cement the casing using 600 sacks of class A cement with 10 barrels of 6% gel spacer ran ahead of the cement and circulation established prior to cementing and maintained throughout the job with cement returns witnessed to surface. The cement mix water PH & Chloride levels were tested.

2016-09-27 | Casing String Witnessed | No Violations | On site inspection to check on the 5.5 casing run issue where the casing string parted during the installation process. Nomac drilling rig 83 is on location and has drilled the 8.5 production borehole to a measured depth of 18,251 and was running their 5.5 production casing with 13,069 inserted into the well bore when the casing parted at a collar 6 joint from ground level - the rig then attempted to spear into the casing pushing it downhole to 1,242. The company then brought in a wireline truck and shot the casing off at 7,248 measured depth and left 5,855 of casing in the hole, the casing will be pulled out of the hole and the company will attempt to recover the remaining casing. ODNR Inspector Brian Sefcik was on site with me on this day.
2016-09-28 | Production Wells | No Violations | Status check inspection on the current 5.5 production casing recover efforts from where the production casing parted while being ran into the well bore. The drilling rig Nomac 83 has recovered part of the casing and is currently ran in the well bore with a pipe spear and hydraulic jars with the casing being encountered at 7,284. The rig was able to get ahold of the casing with conformation of having ahold of the casing by the additional string weight. The rig was circulating the well bore through the entire consist and was beginning to pull out of the hole with a undetermined amount of the casing. Upon contacting the rig on the next working day 09/29/2016 I was informed all casing was recovered and they were going to run back to bottom with the drill string and confirm the hole condition was still good and then pull the drill string and run a new casing consist.
2017-03-10 | Completion Testing | No Violations | Status check inspection on the well completion process found Hybrid Drilling rig 102 completion rig on location rigging up on the well preparing to drill out the completion plugs. A Red Dirt Energy well head master valve is in place along with Hybrid Drillings completion blow out preventer stack. The rig will run 2.875 production tubing with a self rotary drill out bit and drill the completion plugs out to total depth. Upon reaching total depth the rig will pull the string up and hang the 2.875 off at a measured depth of approximately 9,645. The well will then be ready for the production flow back stage. The completion rig is placed on polymer containment. The flow back tanks are also placed on separate polymer containment.
