0748220929 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Hybrid Services completion rig # 102 is on location drilling out the completion plugs on the Roger # 4-A well. The rig is situated on polymer containment with the well pad perimeter earthen berm serving as the primary containment with the pad drain valves closed. There is 6 total wells on the pad that have been drilled and stimulated that will now be completed and the initial flow back operations commenced.
0796922730 2017-06-27 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well site construction has vegetation established and erosion control in place and is in compliance with ORC 1509.072. Addressing the well site. There is 6 producing wells on the pad all in line and selling product with tubing well heads installed on each well and the production valves in the open position. The wells are selling into gas processing units (GPU) with (6) units present. Also on the well site is the production fluid storage facility consisting of (4) steel production tanks all located inside a polymer lined steel containment dike area. The company has installed electrically controlled emergency shut down devices at each well head and also at the production facility and the well pad entrance. Located along the well site entrance lease road is a mid stream operation owned by Rice Energy in which all the wells on the pad sell into. Signage is posted at the well pad entrance and at each well head.
1565251701 2016-03-17 Casing String Witnessed No Violations The said well was spudded on 03/16/2016 with a 110 x 36 conductor borehole being drilled by Butches Rat Hole Service and -110 of 26 .375 wall-weld connection conductor pipe was set and cemented by Stingray Cement Service using 490 sacks of Class A cement with 2% calcium and .125 lb/sk lcm additives. Circulation was established prior to cementing and maintained throughout the job with cement displaced with 62 barrels of fresh water and cement returns witnessed to surface

Nomac Drilling rig # 83 is on the location and has drilled a 17.5 borehole to a depth of 1,345 and ran 13.375 54.4lb/ft casing to a depth of 1,345 with a float collar at 1,299 and guide shoe at 1,345 and proper centralizer placemen demonstrated. Stingray cement service was on location to cement the casing using 1,185 sacks of Class H cement with 25 barrels of 6% gel spacer ran ahead of the cement and circulation established prior to and maintained throughout the cement job with cement returns witnessed to surface and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug.
1865623479 2018-03-15 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The pressure gauges are labeled with surface pressure reading 0 pounds per square inch, and the intermediate gauge reading 0 pounds per square inch. There are (6) total wellheads, all in production, with concrete barriers surrounding them, and an electronic shutdown button at each wellhead. The facility is gated with proper signage, and an electronic shutdown button on the left-side, prior to the pad. The back right-side of the pad is equipped with (4) 400 barrel steel tanks and (1) 50 barrel steel tank all within containment, and an electronic shutdown button is present. There are (6) gas processing units along the right-side of the pad with electronic shutdown buttons on each, and a midstream facility on the right, at the entrance.
