0678998675 2018-07-12 Production Wells No Violations Status check inspection found the well is equipped with a 20 diameter conductor casing string a 13.375 diameter surface casing string 9.625 diameter intermediate string and a 5.5 diameter production casing string. The well head assembly consists of a lower A section and upper B section with a the well producing off the 5.5 diameter production casing string. The well is producing through the production tree with 2 manual valves present on top of the well head B section. Atop of the completion stack is a production flow cross with 2 additional manual shut down valves present and 2 emergency shut down valve also in line. The well is selling into a 3 diameter sales line with a line pressure of 1,286 pounds per square inch line pressure (PSI). The 5.5 diameter production casing pressure is 1,693 psi. Annular pressure monitoring is in place with 500 psi on the 13.375 diameter surface casing and 0 psi on the 9.625 intermediate casing pressure. The well is selling through a flow back manifold with a sand trap vertical separator present. Down line from the sand trap separator is a gas processing unit followed by a contact tower and glycol heater. There is a 3rd party midstream valve point on the West side of the well pad. The production fluid facility consists of 3 steel construction tanks of 400 barrel capacity each, all tanks are situated inside a polymer lined steel containment dike area. The well pad perimeter does have an 8 chain link fence in place and a lock on the gate. I have also addressed a complaint identified as complaint # 5883 filed by a local landowner indicate a odor in the area of her home. I have spoken with a Edgemark representative identified as Jeff Dierdorf in regards to the complaint and also followed up on location.
1088557091 2017-04-24 Completion Testing No Violations Status check inspection on the well completion process found the well drilling and stimulation process complete and the well has the completion plugs drilled out. Colter Energy Services is on location flow testing the wells initial production. The well is being produced through the 5.5 production casing with (2) manually controlled master safety valves present and sold into a production sales line. The preliminary Restoration is due however construction is currently taking place on the well pad to the extent of the production flow lines being installed.
1460190035 2016-04-09 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job. H&P Drilling rig # 521 is on location and has drilled a 17.5 surface borehole to a measured depth of 1,910 off of a 26 rig Kelly bushing and ground level elevation of 955. 1,880 of 54.5 lb/ft J-55 Buttress thread casing was ran with a float shoe and collar used and proper centralizer placement demonstrated. The company geologist verified the casing point was not set into the Berea Sand formation. Halliburton Cement service was on location to cement the casing using a lead and tail class A cement with 50 barrels of 6% gel spacer ran ahead of the cement with circulation maintained throughout the cement job and cement returns witnessed to surface and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug.
1460508128 2016-04-12 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 531 on location currently pulling out of the hole with the drill string after reaching a total depth drilled of 5,762 using a 12.375 drill bit. The rig will pick up a 12.250 and directional drilling assembly and resume drilling the intermediate well bore section to a estimated depth of 8,800-approximately. The rig and support equipment is located on a polymer containment area with the well pad perimeter being the primary containment.

1548427218 2019-01-25 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. There are (2) producing wells, (2) gas processing units (GPU), and (3) 500-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 600 psi.
1946069586 2017-06-07 Production Wells No Violations Status check inspection as a response to complaint number 5619 indicating a that there is a odor coming from the well pad. The company was contacted regarding the complaint and has attempted to address the complaint by decreasing the glycol heater running temperature in attempt to subside the vapor emission from the heater stack. Upon my inspection it was observed that 2 well heads are present on location along with 2 glycol heaters. The production valves were in the open position and selling into the glycol heaters. Also on location is a production facility consisting of 3- 400 barrel capacity steel tanks located inside a polymer line steel containment dike area. A slight odor was present on location but not noticeable in the surrounding area as before the complaint.

2136270827 2019-05-21 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072(A). I found the site gated with signage at the entrance and surrounded with an earthen berm. There are (2) producing wells and (2) gas processing units. The production facility consists of (3) 500 barrel steel tanks all within containment. Annular pressure monitoring devices are equipped. The 9.625 inch diameter casing has a pressure of 0 pounds per square inch (psi) and the 13.375 inch diameter casing has a pressure of 700 psi. Identification is posted at the wellhead, along with an emergency shutdown button. It will be noted that I spoke with EdgeMarc Production Superintendent Mark Toplovich onsite.
