0500717366 2017-01-03 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Well Service Group rig # 1 on location and a 26 surface borehole drilled to 545. The rig then ran 515 of 20 diameter 94 pound per foot J-55 casing with a double valve sting in casing float shoe ran. Halliburton Cement Service cemented the casing by 1st establishing circulation to surface and then pumping 40 barrels of 6% gel spacer followed by 830 sacks of class A cement with cement returns witnessed to surface.

1526993994 2018-05-22 Production Wells No Violations On site inspection found the well drilling and completion process now complete and well in production. The well is selling off the 5.5 diameter production casing through a primary 7.187 master valve with 2 additional in line manual and 2 hydraulic shut down valves in the production flow line. The company has annular pressure monitoring in place with riser pipes and gauges on the 13.375 and 9.625 well head sections. The 13.375 casing well head section gauge showed 0 pounds per square inch (psi) and the 9.625 well head section had 3,800 psi. Also on site is a total of 3 gas processing units one for each well on the pad along with a production facility consisting of 2 steel construction 400 barrel production tanks situated inside a polymer lined steel containment area. With the wells in the initial flow back stages the company has an additional 8 steel 500 barrel tanks that are being used to capture the flow back fluid from the wells, the 3 wells in production is generating approximately 800 barrels of fluid total each day. The preliminary restoration is also due- no earthen slips or deficiencies was observed on this day, vegetation is present throughout the disturbed area depicted on the initial construction design. The lease road is in good repair and consists of a gravel surface. The well pad entry point is gated with signage present. I will follow back up on the restoration later in the year and re confirm proper vegetation is still present. I also called the company and asked them to address the mouse holes used in the drilling process that have not been plugged and address the 9.625 diameter x 5.5 diameter annular pressure.
