0250402340 2017-06-27 Preliminary Restoration No Violations Preliminary Restoration status check found the area affected by the well site construction has vegetation established and erosion control in place. There is one producing well on the pad with a tubing head installed and the production valve in the open position. Annular monitoring is in place. The well is selling into a gas processing unit and then flows off site to a midstream processing facility owned by Rice Energy. Also situated on the well pad is a production facility consisting of (4) steel 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. The well pad perimeter earthen berm is in good repair. There is signage posted at the well pad entrance and at the well head. The company has also installed emergency shut down devices at the well head and the production facility along with at the well pad entrance.

0867536331 2018-03-05 Production Wells No Violations Producing well status check inspection found the well head designed to produce from the tubing well head above the B-Section and casing head section. In the tubing section vertically there is a total of (2) manually operated valves then the tubing flow cross, attached to the tubing flow cross downstream is a third manual valve then a Emergency Shut Down (ESD) valve with remotely controlled switches at multiple locations on location including at the well head and gas processing unit along with the production facility and entry point of the location. The said well has annular pressure gauges in place with the valves in the open position. The annular pressure between the 13.375 casing and 9.625casing reads 0 psi and between the 9.625 casing and the 5.5 casing 750 psi. Addressing the well support equipment on location- at the entry point of the location is a midstream operation operated by EQT Company, on the left hand side of the well pad is a gas processing unit At the right rear corner of the well pad is the production facility consisting of a total of (4) steel construction tanks each with a total fluid volume of 400 barrels capacity, the tanks are situated inside a polymer lined steel containment dike area. The well pad does have a perimeter earthen berm. Proper well identification is posted at the well and the tank battery as well as the entry point of the lease road and well pad.

The stage was perforated by Renegade Wireline service with a completion plug set at 14,308 and the well perforated between the depths of 14,308 and 14,147 with 50 perforations shot in 5 clusters. Red Dirt Energys 15,000 psi completion isolation stack is being used. The well pad perimeter earthen berm is being used as primary containment with the drain valves in the closed position and all stimulation equipment is placed on polymer containment.

1474705858 2016-09-24 Casing String Witnessed No Violations 09/24/16- H&P 628 is on location and has drilled a 17.5 surface borehole to a measured depth of 1,220 with 13.375 casing ran and set to a depth of 1,220 with a guide shoe- float collar and 12 centralizers casing equipment ran. Stingray Cement service cemented the casing using 1,220 sacks of class A cement with the cement weight verified by weight of scale and 1,063 sacks of class A cement ran with 25 barrels of 6% gel spacer ran ahead of the cement and circulation established to surface prior to cementing. The cement was displaced to the float collar with 179 barrels of fluid with the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug. Cement returns were witnessed to surface inside the cellar and the cement level left at 5 in the cellar to create a liquid tight cellar.
