1026392942 2017-01-24 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area disturbed within the well pad limits of disturbance has been restored according to ORC 1509.072 with erosion control in place and vegetation established. The wood chip filled filter sock used around the well pad perimeter to control erosion is still in place. The outward slopes of the well pad are stable and the lease road is in good repair, the well pad entrance is gated and signage is posted. Addressing the 4 wells present on the pad, the well drilling is complete and the wells are awaiting stimulation. No production equipment is present.
1095092135 2018-05-02 Production Wells No Violations status check inspection found there is 3 wells now drilled and producing on the pad. Each well is producing off the production tree and the production valves are in the open position, the production flow line valve shows 2,000 psi. There is annular pressure monitoring in place, the 13.375 surface casing well head section has 20 psi on the gauge and 750 psi on the 9.625 well head section gauge. The well is equipped with an emergency shut down button that is controlled from multiple point on the well pad. There is a production facility present consisting of (4) steel 400 barrel tanks situated inside a polymer lined steel containment area.
1475711033 2016-10-05 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job found Lighting top hole services rig on location and has drilled a 36 borehole to 110 and ran 110 of 26 casing. Stingray Cement Service cemented the casing using 500 sacks of class A cement with circulation established prior to cementing and 15 barrels of 6% gel spacer ran ahead of the cement and the cement displaced with 61 barrels of fresh water.
1476958294 2016-10-20 Casing String Witnessed No Violations H&P rig 628 is now on the hole and has drilled a 17.5 surface hole to 1,338 and ran 13.375 casing to 1,338. Stingray cement service cemented the casing using 1200 sacks of class A cement with the well not circulating until 135 barrels into a total of 198 displacement. The casing was pressure tested to 500 psi over cement displacement pressure upon the landing of the cement plug.
1832199704 2018-06-05 Production Wells No Violations On this day, I found the well equipped with annular pressure monitoring devices. The 13.375 inch diameter surface casing pressure gauge is reading 40 pounds per square inch, and the 9.625 inch diameter intermediate casing pressure gauge is reading 500 pounds per square inch. The facility is gated with proper signage and an electronic shutdown button at the entrance. There is a midstream facility at the front left-side of the pad, prior to the gate. (3) gas processing units are located at the front right-side. There are (3) wells in production on this pad, all with electronic shutdown buttons. All wells have concrete barriers surrounding them. The wellheads are properly identified. There are (4) 400 barrel steel tanks within containment, at the back right-side, with an electronic shutdown button.
