0528098534 2018-06-20 Production Wells No Violations Status check inspection found there is 4 producing wells on the pad. All wells have had the drilling and completion process complete and are in production. The well are producing off the production tree with emergency shut down devices in line. Annular pressure monitoring is in place with 1,015 psi showing between the 5.5 and 9.625 and 200 psi showing between the 9.625 and 13.375. On site is 4 gas processing units and a production facility consisting of 4 steel tanks capable of holding 400 barrels capacity each. The tanks are situated inside a polymer lined steel containment dike area. . I spoke with Consol regarding clarification on the pressure readings on the well head annulis.

1048429599 2017-02-15 Drill / Deepen / Reopen No Violations On 2/14/17 I received a call from CNX reporting a small release. An estimated 4 barrels of oil base mud was released through the flare stack on to the pad. Immediate clean up took place; the contamination was removed. During my inspection I witnessed new gravel being applied to the area where the release occurred. Patterson Drilling Rig 803 is currently installing the night cap and preparing to walk to the 11 L.
1485576977 2017-01-27 Casing String Witnessed No Violations On site inspection to witness the surface mine string combination cement job with Patterson Drilling rig 803 on the hole and Allied Cement Service along with a Blackhawk oil tools representative. Patterson Drilling has drilled a 24 borehole to 751 with a coal seam encountered at 645 to 660. The rig then ran 691 of 18.625 J-55 casing with a casing guide shoe ran on bottom and a TAAM swell packer and Davis Lynch stage tool set in the string at 422. Allied Cement Service cemented the casing by 1st pumping 200 barrels of fresh water followed by 50 barrels of 6% gel spacer followed by 490 sacks or 98 barrels of class A cement with 2% calcium and 1% gel cement additives. The 1st stage cement was displaced with 210 barrels of fresh water and upon the landing of the cement plug casing pressure was increased to inflate the swell packer followed by the stage tool being opened mechanically. Circulation was then established to surface above the swell packer and stage tool and the upper section of the string cemented with 700 sacks of class A cement with 2% calcium and 1% gel additives, the cement was displaced with 128 barrels of fresh water and the casing was pressure tested to 500 psi over cement displacement pressure upon the landing of the plug with cement returns witnessed to surface.
