1015076485 2017-02-18 Casing String Witnessed No Violations I was on location to witness Stingray Cementing cement the 24 inch diameter casing into a 30 inch diameter borehole. Pioneer drilled a 120 foot 30 inch diameter borehole and set 120 feet of 24 inch diameter casing into the hole. Stingray Cementing ran 15 barrels of fresh water gel ahead of the cement. Stingray Cementing then ran 290 sacks of class A cement, 2% CaCL, and .25 parts per sack of floseal with 62 barrels of fresh water at 15.6 pounds and a yield of 1.2. Stingray Cementing then displaced with 57.5 barrels of fresh water until we got cement to surface. Stingray Cementing circulated about 10 barrels of cement to surface and finished cementing the casing into the bore hole
1491600392 2017-04-07 Casing String Witnessed No Violations On site to witness the surface casing cement job with H&P Drilling rig 608 and Schlumberger Cement Service. The rig has drilled a 17.5 diameter surface borehole to a measured depth of 351.39 and ran 13.375 casing in the hole to a measured depth of 313 with a drilling rig Kelly bushing height of 27 from ground level. Schlumberger Cement Service Cemented the casing using 83 barrel class A cement mix consisting of 385 sacks of cement. The cement weight was verified by weight of scale at 15.6 pound per gallon and had a yield of 1.22. The cement company ran 20 barrels of 6% gel spacer ahead of the cement and displaced the cement with 41 barrels of recycled water and the plug landed at 130 psi. The casing was pressure tested to 900 psi with 15 barrels of cement returns witnessed to surface.
1816285661 2018-08-21 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with erosion control and vegetation established. The lease road and well pad is of stone construction and in good repair. The well pad perimeter is protected by an 8 chain link fence with signage posted at the entrance point. There are 5 wells present on the pad 3 Utica and 2 Marcellus. Each well is properly identified with signage. Fire extinguishers and emergency shut down devices are present at all critical location points on the well pad. Addressing the well - The well is equipped with a lower A and upper B section with a production flow cross incorporated into the B section on the Marcellus wells. The well is equipped with a production tree consisting of a manually operated primary master valve and a emergency shut down valve. Present on the well head A & B sections are 2 diameter riser pipes equipped with valves and gauges to monitor annular pressure. The A section gauge that measures the pressure between the 9.625 and 13.375 diameter casing strings shows zero pressure and the B section gauge which measures the pressure between the 5.5 and 9.625 diameter casing strings measures 1,100 pounds per square inch (psi) pressure. The 5.5 diameter production casing internal working pressure is 9,00 psi and the production flow line is 500 psi. Production equipment on site includes multiple line heaters a contact tower and glycol heater along with a gas compressor. The production fluid storage area consists of (10) steel construction 400 barrel fluid capacity tanks situated inside a polymer lined steel containment dike area with well and hazard identification present along with fire extinguishers and a emergency shut down control point. The company has also added an additional production fluid storage area consisting of (5) steel construction 500 barrel fluid capacity tanks.
