0282381306 | 2017-08-24 | Completion Testing | No Violations | On site inspection to witness the well stimulation process found Universal Well Service pressure pumping service on location stimulating stage 21 of 38 total stages to be stimulated. The stage was stimulated with a total of 11,766 barrels of fluid with a top treating pressure of 10,147 psi. The pressure pumping equipment was pressure tested to 11,600 psi prior to the stage. The stage was perforated between the depths of 14,465 and 14,312 with 48 perforation shot in 4 clusters of 12 shots each cluster. A completion plug was set at 14,485. No abnormal annular pressure was recorded over 25 psi during the stage. The stimulation support equipment is placed on polymer containment with the well pad perimeter earthen berm serving as the primary perimeter containment. |
0385230817 | 2018-02-21 | Production Wells | No Violations | Status check inspection found the well drilling and completion process now complete and the well in line and producing from the tubing flow cross section. The casing section of the well head is isolated with no flow lines attached. All well head annulis have pressure gauges and flow lines present with check valves present on the flow lines. On location is also a production fluid tank battery consisting of a total of (4) tanks of steel construction design situated inside a polymer lined steel containment area. Also on location is 4 gas processing units on each servicing each well on the pad. EQT also has a midstream operation off the back corner of the well pad. Emergency Shut down devices are located at the well pad entrance-at each well head -the tank battery and the gas processing units. |
0850832311 | 2018-05-21 | Production Wells | No Violations | Producing well status check found that a total of 4 wells are present on the well pad and in production with the production valves on the tubing head assemblies in the open position. There is a total od 4 gas processing units on location and 4 steel design production tanks. A 3rd party midstream operator identified as EQT is set up at the rear of the well pad. There is a large portable water storage tank used in the stimulation process set up on the well pad also. |
1350784905 | 2017-08-16 | Completion Testing | No Violations | The pad is named Switz 26 and consists of 4 wells. During my inspection Universal Pressure Pumping, Allied Wireline, and CNX Company Man were on location. Universal Pressure Pumping was rigged up for hydraulic stimulation operations on the C and A well. Operations were at a stand-still and there was not any equipment in operation on location. I discussed with CNX Company Man annular pressure monitoring. He stated that they monitor the annular pressure at the wellhead and then run a line to the frac van when operations begin to record and monitor their pressures. The wellhead was equipped with a frac stack and goat head assembly with all valves in the close position. Annular pressure monitoring was present. A 2 inch diameter steel line was plumbed up to the 9.625 inch diameter casing and ran to a bled off tank. |
1435687518 | 2017-02-17 | Casing String Witnessed | No Violations | I was on location to witness Express Energy cement the 30 inch diameter casing into a 36 inch diameter borehole. Express Energy drilled a 100 foot 36 inch diameter borehole and set 100 feet of 30 inch diameter casing into the hole. Express ran 16 barrels of fresh water gel ahead of the cement. Express then ran 275 sacks of class A cement, 2% CaCL, and .5 parts per sack of floseal with 57.79 barrels of fresh water at 15.6 pounds and a yield of 1.18. Express then displaced with 74.8 barrels of fresh water until we got cement to surface. Express Energy circulated about 20 barrels of cement to surface and finished cementing the casing into the bore hole. The cement fell 30 feet from the surface of the conductor. The well was grouted from the surface the same day it was cemented to fix this issue. |
1495037238 | 2017-05-17 | Casing String Witnessed | No Violations | The purpose of my inspection was to witness the cementing of 677 feet of 18.625 inch diameter surface casing with in a 24 inch diameter hole drilled to a depth of 704 feet. During my inspection I witnessed Allied Oil Services attempted to pressure up to pressure test the lines and the pump malfunctioned and would not work properly. An estimated time of 5 hours was given for the arrival of the replacement pump truck. |
1501769037 | 2017-08-03 | Completion Testing | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a frac valve in the closed position. The pad surface was lined with containment liner and frac tanks were being placed on containment. Location was in preparation for hydraulic stimulation operations. |
1657395091 | 2017-05-17 | Casing String Witnessed | No Violations | The purpose of my inspection was to witness the surface-mine string cement job. The coal seam is from 623 feet to 639 feet in depth. Patterson Drilling Rig 803 drilled a 24 inch diameter borehole to a depth of 704 feet. The 18.625 inch diameter casing, 87.5 pounds per foot, was run to a depth of 677 feet. Following previous problems with a pump truck Allied Cement Services was on location with a functioning pump truck to cement the casing string in two stages with a packer set at 400 feet. 150 barrels of fresh water, 50 barrels of gel, and 20 barrels of fresh water were pumped ahead of the cement. 79 barrels of Class A cement with 1.5 percent calcium and 206 barrels of displacement were pumped to complete the first stage. The plug landed and the packer was energized. The second stage was completed using 60 barrels of fresh water, 50 barrels of gel, 15 barrels of fresh water, and 137 barrels of Class A cement with 1.5 percent calcium. 110 barrels of displacement were pumped, the floats held, and 14 barrels of cement returns were witnessed in the cellar. Cement density from both stages was checked during the job using pressurized scales confirming the weight of 15.6 pounds per gallon. |
