0655868364 | 2017-10-03 | Completion Testing | No Violations | On the day of my inspection, I accompanied ODNR Engineer Technician Andrew Thomas. We were on location to examine a fluid release that occurred. Upon inspection, we found Halliburton on location completing the wells and S3 Pump Service running logging tools down hole. Also, we observed cleanup efforts for recent release. It was indicated that flange bolts broke on the high-pressure side of a pumping unit and released flowback fluid on containment and the pad surface. It was stated that the release contained approximately 40 barrels of flowback fluid. Contaminated materials are being dug up and stockpiled, then hauled from location in roll-off bins by Cowboy Industries. The materials will be taken to a facility to be tested. Containment liner beneath the frac trucks was ripped and indicated seepage to the pad surface. Small trenches are being dug to assist in the collection and removal of fluid. More environmental investigation may be needed to assess total impact to the pad surface and sub-surface. Bruce Carpenter was contacted by Andrew Thomas and explained the situation. Bruce said he would contact ODNR when they would do the investigation. |

1488865933 | 2017-03-07 | Casing String Witnessed | No Violations | On site inspection to witness the surface casing cement job arriving at 23:00 hours on 03/06/2017 to attend the safety meeting and witness the 13.375 surface casing cement job with H&P Drilling rig 608 on location along with Schlumberger Cement service contractors. Upon arrival I was informed the rig was unable to land the last 2 casing joints due to hole conditions and would lay one casing joint down and load the hole with fresh water and circulate the last joint in the hole. The rig had drilled the 17.5 diameter production borehole to a measured depth of 1,872 and ran a 13.375 split weight casing string with 547.8 of 61 lb/ft J-55 grade casing on the bottom of the string and 1,231.31 of 54.5 lb/ft J-55 buttress thread on the upper section of the string with a total of 1,777.11 of 13.375 casing ran with a float shoe and float collar casing equipment ran. At 4:45 am on 03/07/17 the casing was landed, the cement head installed and the safety meeting held. The cement job was began at 5:00 am by pumping 20 barrels of 6% gel spacer followed by 320 barrels cement consisting of 1,485 sacks of class A cement. The cement was displaced with 267 barrels of fresh water and the casing was pressure tested to 500 psi over cement displacement pressure upon the landing of the cement plug in the float collar. Cement returns were witnessed to surface. |

Stingray Cementing then displaced with 57.7 barrels of fresh water until we got cement to surface. Stingray Cementing circulated about 15 barrels of cement to surface and finished cementing the casing into the bore hole. |

The 5.5 diameter production casing internal working pressure is 1,500 psi and the production flow line is 500 psi. Production equipment on site includes multiple line heaters a contact tower and glycol heater along with a gas compressor. The production fluid storage area consists of (10) steel construction 400 barrel fluid capacity tanks situated inside a polymer lined steel containment dike area with well and hazard identification present along with fire extinguishers and a emergency shut down control point.
