Onsite inspection to witness the conductor casing cement job with Express Energy Services. The well was spudded on 1/28/18. The rig drilled a 36-inch diameter borehole to a depth of 100 feet from ground level. Then, 100 feet of 26-inch diameter weld connection casing was inserted into the borehole. They cemented the casing with 120 barrels total volume of 573 sacks of Class A cement with a 1.18 yield, 2 percent calcium chloride, 0.5 pound of flake per sack, and displaced with 56 barrels of fresh water. No cement returns at surface. Cement level is approximately 40 foot down hole. Caldwell Redi-mix grouted with 10 cubic yards down the annulus and back up to the cellar. Cement returns witnessed at surface. |

The well was stimulated in 83 stages with a single isolation plug set in the 5.5 diameter production casing at a depth of 8,831. A tubing snubbing unit owned by Deep Well Services was on location and rigging up on the Loraine #1-H. The snubbing unit identified as unit # 6 will drill out the completion plugs on all 6 wells on the pad and prepare them for production. The completion well control blow out preventer is owned by KLX Energy. The pressure pumping service is will be provided by Key Energy with a Sand X system for filtration of the well bore materials returned to surface. The completion equipment is situated on polymer containment with poly mats placed on the containment. The well pad perimeter drain valves are in the closed position. Production equipment is present on the pad. Also noted on this day I spoke with the Antero Company Representative on site Bryan Carter and we also called a Doug Phenix regarding the absence of annular pressure monitoring on the well head A&B sections. The men were informed of ORC 1501-9-1-08 section (5) of the well construction rules regarding annular pressure monitoring. |

The rig has drilled a 17.5 diameter surface borehole with a hammer bit misting on air to a measured depth of 1,215 off a drilling rig Kelly Bushing heigth of 30 above ground level. Upon reaching total depth the rig pumped several soap sweeps to condition the hole and pulled the drill string. Wyoming Casing inserted the 1,205 of 13.375 diameter 54.5 pound per foot J-55 buttress thread casing with a float shoe and guide collar casing equipment ran along with 8 casing centralizers. Schlumber Cemented the casing by first pumping 225 barrels of fresh water to establish circulation followed by 30 barrels of 6% gel then starting the cement mix down hole consisting of 229 barrels of a single slurry lead cement with the cement consisting of 1,079 sacks of class A cement weighing 15.6 pound per gallon by weight of cement. The cement was displaced with the float shoe with 180 barrels of fresh water. The casing was pressure tested to 1,098 psi with the final displacement pressure being 548 psi. Cement returns were witnessed to surface inside the cellar at 125 barrels into displacement The well head B section assembly is a Cameron 10,000 psi rated thread on well head built off the 13.375 casing. |

The production tank farm is built and consists of (10) steel construction 400 barrel capacity tanks that are situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm with multiple sumps and drain valves situated on each side of the well pad. |
