0261095216 2017-11-01 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has vegetation established and erosion control in place. The lease road and well pad is of stone construction with the well pad entrance gated. Signage is present at the lease road entrance off of State Route 26. There are 6 wells on the pad that has been drilled but not completed. No production equipment has been placed on the well pad at the time of my inspection.
0623237556 2019-03-06 Drill / Deepen / Reopen No Violations On this day, I found the pad gated with signage at the entrance. There are (6) wells on the pad that have been drilled, but not completed. Steel fencing surrounds the perimeter of the wellheads. Each well has annular pressure monitoring devices equipped. Also noted, each well has signage stating that a back pressure valve (BPV) is installed. The 9.625 inch diameter intermediate casing has a pressure of 400 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. I spoke with Gulfport Representative Josh Starcher during the inspection. It was indicated that the wells may be completed in 2020.

1495065968 2017-05-17 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job with Rocky Mountain Drillings top hole rig and Stingray Cement Service. Rock Mountain has drilled a 32 diameter borehole to 110 from ground level and ran 26 diameter .375 wall weld connection casing. Stingray cemented the casing using a 61 barrel cement mix consisting of 280 sacks of class A cement with 40% open hole cement excess ran. The cement weight was verified by weight of scale and displaced to 100 with 62 barrels of fresh water. 15 barrels of 6% gel spacer was pumped ahead of the cement.15 barrels of cement returns were witnessed to surface.
1501176751 2017-07-27 Casing String Witnessed No Violations The purpose of my inspection is to witness 1055 feet of 13.375 inch diameter surface casing being cemented within a 17.5 inch diameter borehole. The casing was 54.5 pounds per foot. When I arrived on location Pioneer Drilling Rig 73 had the casing landed and was waiting on the arrival of O-Tex Cementing. Cement calculations were discussed with the Gulfport Company Man and a copy of the casing tally was obtained. I witnessed O-Tex Cementing rigging up prior to a safety meeting that was held with all personnel on location including O-Tex Cementing, Pioneer, and Gulfport Company man. Following the safety meeting O-Tex pumped 50 barrels of gel and 207 barrels of cement. The cement was Class H cement weighing 15.6 pounds per gallon. I witnessed O-Tex pull a cement sample and weigh it using pressurized scales to confirm the weight of 15.6 pounds per gallon. The plug was dropped and 157.2 barrels of fresh water was used for displacement. An estimated 35 barrels of cement returns were witnessed in the cellar.

1640991818 2019-03-07 Drill / Deepen / Reopen No Violations On this day, I am accompanied by ODNR Southeast Supervisor Brent Bear to conduct a follow-up inspection in regard to annular pressures. The current pressure reading on the 9.625 inch diameter casing is 400 pounds per square inch, and the pressure on the 13.375 inch diameter surface casing is 0 psi.
