The rig drilled a 8.750 diameter borehole on air to 820. The rig then began running the 7 casing string with 19 casing joints 42 in length. The rig was able to run casing to the mid way point of the 14th casing joint at an approximate depth of 620 the casing would not go past that point. The rig then began pulling out of the hole and laying down casing. Once the casing was out of the hole the rig owner Kerry Brown indicated they would stop work for the day and start again on 09/21/2018. |

My inspection found the site to have insufficient vegetation established within the affected area. The site is not in accordance with ORC 1509.072(A). The well is equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. The production facility consists of (1) 100 barrel steel tank, (1) 100 barrel polymer tank, and (1) 50 barrel polymer tank all contained within an earthen dike. A vertical separator is also located nearby. Additionally, there are (11) frac tanks currently on location awaiting to be moved to the next drilled well. Several tanks contain produced fluid. Standing fluid was tested at the base of a tank with a High Range Quantab with a result of 2,359 parts per million chlorides. I spoke with Mr. Dean Christman on 04/22/19 to notify him of the potentially leaking tank, and advised to clean the affected area, transfer the fluid from the tank, and eliminate future use if not addressed. Mr. Christman will have 30 days to address the restoration by establishing vegetation with no substantial erosion. |

The rig is drilling a 12.250 conductor borehole to a proposed measured depth of 120. The rig picked up surface water at 15 and is having trouble keeping the hole free of water and clean due to the heavy water influx. The well bore was drilled to 120. The rig pulled the drill string and iuserted 120 of 29 pound per foot 9.625 diameter casing. The casing was cemented by Formation Cementing using a 10.5 barrel absolute volume cement mix consisting of 50 sacks of class A cement. Circulation was established prior to cementing and 6 barrels of cement returns were witnessed to surface. Displacement was 6.7 barrels of fresh water. |

Initially on 09/20/18 an 8.750 diameter surface borehole was drilled to 820 and 7 diameter casing was attempted to be ran unsuccessfully with restriction being encountered in the well bore at 620. The drilling rig company identified as Monroe Drilling pulled the 7 and reamed the well bore back to bottom before re running the 7 and having Formation Cementing cement the casing using a 135 sack class A cement mix. On this morning the rig was picking up their production borehole drill bit and was preparing to begin air drilling the production borehole to a proposed depth of 4,000. Both the cement ticket and casing tally for the 7 was received on this day. Also noted is on this day I contacted both Kerry Brown with Monroe Drilling and Dean Christman the operator and notified them that according to the well permit application the uppermost target formation is the Berea Sand and according to 1501:9-01-08 section (7) of the well construction rules the calculated top of cement during the cementing of the 4.5 diameter production casing must be 1,500 true vertical depth from ground level elevation. |
