0679914793 2017-09-14 Casing String Witnessed No Violations On the day of my inspection, I accompanied ODNR Inspector Austin Gutridge and witnessed the cementing of 120 feet of 24 inch diameter conductor. Pre-job safety meeting was held with all personnel on location including Precision Rathole crew, Stingray cement crew, and Eclipse. Stingray pumped 10 barrels of water, 62.5 barrels of cement, and displaced with 57 barrels of fresh water. Cement returns were established in cellar.

2032948391 2017-09-06 Not Drilled No Violations On this day I met on site with Bruce Carpenter with Eclipse Resources and members of the US Beau of Land Management (BLM). The well pad construction certification has been approved by the Divisions Engineering staff and the well drilling permits have been issued. The Company wishes to spud the wells however the (BLM) has indicated due to (2) of the wells on the pad are being drilled into a Federal Mineral a Federal Drilling permit must be issued and other Federal Permit Requirements regarding the well pad construction must be met before the wells can be spudded. Also noted is the well pad location is on private and not Federal property.
