0302208402 2018-07-09 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has erosion control in place and vegetation established. The said well drilling and completion process is complete. The well is equipped with annular pressure monitoring with 1,200 psi showing on the 13.375 surface casing gauge and 2,800 psi on the 9.625intermediate casing gauge. The mouse hole from the drilling rig will need filled with cement. Signage is posted on the well head and at the gate with well control shut down devices at multiple points on the well pad. There is 2 gas processing units present at the rear of the well pad supporting each of the 2 producing wells on the pad also present is a production fluid tank battery consisting of a total of 3 steel construction 400 barrel capacity tanks present inside a polymer lined steel containment dike area. The tanks have the dome lids secured and closed with 12 ounce pressure relief devices on the production tanks that are releasing into a 100 barrel tank that is set on location away from the other production equipment, no production tanks are equipped with any low pressure recover system. I discussed with the production man on location identified as Adam Mallet the need to install a low pressure recover system and not allow the vapors or gases to be released into the atmosphere. Also discussed was the lack of a gauge or any type of annular pressure monitoring on the 20 diameter string identified as the surface casing string. Also discussed was the need to release the annular pressure off the 13,375 and 9.625 casing strings.
1277631519 2017-12-27 Drill / Deepen / Reopen No Violations Status check inspection on the current well drilling progress found H&P Drilling rig 611 on location. The rig is currently drilling the 12.250 2nd intermediate well bore in the Oriskany formation at a measured depth of 6,836. The rig has pulled the PDC drill bit for a bit change. They are attempting to deviate around a lost drill bit and are drilling on 9.5 pound per gallon brine water. The 1st 12.250 well bore was plugged back to 5,775 at which point the rig deviated off the cement plug and began to drill around the lost well bore.
1512537793 2017-12-06 Casing String Witnessed No Violations Status check inspection to witness the surface casing cement job arriving at 21:00 hours on 12/05/2017. H&P Drilling rig 611 is on location and rigged up over the well, the rig has drilled a 24 diameter surface borehole on air to a measured depth of 395 off of a drilling rig Kelly Bushing heigth of 25 above ground level elevation. Upon reaching total depth the rig pulled the drill string and inserted 20 diameter J-55 grade casing weighing 94 pound per foot with a casing guide shoe and float collar ran as part of the 45 casing shoe track, 5 casing centralizers were also ran to provide proper casing stand off. CJ Energy Service cemented the casing using a 117 barrel absolute volume cement mix consisting of 550 sacks of class H cement with the cement weight being verified at 15.6 pound per gallon by weight of scale. The cement was displaced to the float collar with 118 barrels of fresh water with a final displacement pressure of 128 pounds per square inch (psi) and the casing being pressure tested to 790 psi, 40 barrels of cement returns were witnessed to surface.
1513274261 2017-12-14 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 611 on location. The rig has drilled the 12.250 diameter 2nd intermediate well bore section to a measured depth of 4,650 and has pulled the drill string out of the hole due to a drill string float failure. The rig has proposed to drill to 7,000 measured depth and then switch over to drilling from air to fluid. The rig and support equipment is placed on polymer containment with the well pad earthen berm serving as the primary containment with the drain valves in the closed position. No environmental issues are present on or around the location.
1515524350 2018-01-09 Casing String Witnessed No Violations On site inspection to witness the 2nd intermediate casing string cement job with H&P rig 611 on location along with C&J Cement service. The rig has drilled a 12.250 diameter well bore to a depth of 10,218 measured off a drilling rig Kelly bushing heigth of 25 above ground level elevation. The rig drilled the last 3,000 of well bore using 9.5 pound per gallon brine water. The Selina salt formation top was encountered at the depth of 7,147 and was caliper logged showing the formation was washed out past a recordable gauge limit. Upon reaching total depth the company conditioned the well bore with 9.5 pound per gallon oil based drilling mud before pulling the drill string and running the 10,204 of 5.5 diameter L-80 grade buttress thread casing. The 88.6 casing shoe track consisted of a float shoe and float collar. Also ran on the casing string was a Wetherford external casing packer set in the string at 7,203 and a cement stage tool also set in the string at 7,195. C&J Energy Service cemented the 2 stage cement job by first pumping 50 barrels of spacer with surfactants mixed at 11.5 pound per gallon followed by a lead cement mix consisting of 185 barrels absolute volume cement consisting of 690 sacks of class H 50% poz mix cement followed by a tail cement mix of 156 barrels absolute volume cement mix consisting of 750 sacks of class H cement. The cement was displaced to the float collar with 749 barrels of fluid. Circulation and 340 pounds of lift pressure was maintained until I left location at 300 barrels into displacement of the bottom cement job at 16:30 pm.
