1134938576 2019-04-16 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072 (A). I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units, (3) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has annular pressure monitoring devices equipped. Greg Davis, Equinor Representative, was notified to label the gauges and wellheads with proper identification. Mr. Davis also stated that annular pressures are monitored daily. It will be noted that a third-party contractor, FMC Technologies, is onsite with secondary sand traps. The production records are current.
1154473965 2018-07-30 Completion Testing No Violations Status check inspection on the well completion process accompanied by ODNR Inspector Tom Thomas along with Darrin Galisky representing Stat Oil. Current operations are the well drilling process is complete and Halliburton pressure pumping along with Allied Horizontal Wireline is rigging up preparing to begin stimulation operations on the 3 wells present on the pad. All stimulation support equipment is situated on polymer liner. The well head completion stack consists of a primary master valve situated atop the B section of the well head followed by a hydraulic master valve then the well head flow cross. Atop the flow cross is a upper hydraulic master valve followed by the pressure lines before the wireline head attachment point. There is a separate accumulator control for each well on the pad. The well completion operations are to begin in the next 48 hours.
1219910264 2018-07-30 Completion Testing No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. Halliburton is in their preparation phase to begin hydraulic fracturing. We spoke with Statoil Field Completions Superintendent, Darren Galisky, and conducted a walkthrough of the pad and equipment onsite.
1534358779 2018-08-15 Completion Testing No Violations Status check inspection on the well completion process found Halliburton Pressure pumping service on location stimulating the 3 wells present and Horizontal Wireline on location providing the wireline service. The last stage treated was stage 27 with a average treating pressure of 10,000 pounds and break pressure of 8,400 pounds per square inch pressure (psi) the initial shut in pressure for the wells on the pad has been averaging 6,400 psi. Horizontal wireline is running in the hole to perforate stage 28 which will be perforated with 6 clusters of 40 perforations total. The stage will be perforated between the depths of 19,827 and 19,993 and a completion plug will be set at 20,010. The company is monitoring all annuli electronically. All stimulation and support equipment is placed on polymer containment.
