0858776276 2018-01-30 Production Wells No Violations On site inspection found a service rig owned by Chuck Dye moving onto the well location to plug the said well. A plugging permit has been issued by the Division and the well location confirmed to match the permit. The well was observed to be equipped with 5.187 diameter production casing and 2.375 diameter non upset production tubing. Pumping rods are inserted into the well bore however no pumping unit is present on the well. No flow lines are attached to the well and both the casing and tubing annulus are standing open to the atmosphere. The well is in disrepair and there are multiple holes present in the 5.187 casing above surface grade level. The annular behind the 5.187 casing is open and wooden conductor is present. The producer Dean Christman spoke with Supervisor Mike Moreland who has requested a written plan from Mr. Christman on how the company intends to plug the well. Mr. Christman indicated he was not sure on how to plug the well.
0880680490 2018-02-06 Plug / Plug Back No Violations On this day, I accompanied ODNR Inspector William Stolarik to witness the final stages of the well plugging process. Please reference his comments for a detailed report. A bottom of surface plug was set with 50 sacks of Class A cement with 1% gel additive. The surface plug was set with 30 sacks of Class A cement and cotton seed hauls, with returns witnessed at surface.
1517610326 2018-02-02 Plug / Plug Back No Violations On site inspection to witness the well plugging process with a service rig on location owned by Chuck Dye and Formation cementing. The rig has pulled the 2.375 non upset production tubing and pumping rods out of the well and ran 2.375 11.5 V thread tubing back in the well as a plugging string to a total depth of 1,523. Formation Cementing pumped a 16.83 barrel absolute volume bottom plug consisting of 75 sacks of class A cement with 1% gel and 1% calcium. The cement yield was 1.26 and weight was 15.2 pound per gallon. Upon reaching total displacement of 2.3 barrels of fresh water the displacement pressure increased from 150 psi to 700 psi and the tubing was immediately stuck in the well bore. No completion records were available before commencing the job and the service rig owner Chuck Dye indicated He felt the pencil cave formation at 1,200 was not covered by the 5.187 diameter production casing and reacted with the fresh water ran ahead of the cement and the formation collapsed.
1517902767 2018-02-05 Plug / Plug Back No Violations On site inspection to witness the well bore bond log evaluation. On this day Appalachian wireline service was on location and ran a bond log on the 2.375 diameter tubing string that became stuck in the well bore after pumping the bottom hole cement plug on 02/02/2018. The bond log showed cement on the back side of the tubing up to 1,150 and the 2 diameter tubing was shot off at 1,000. Immediately upon shooting the tubing the fresh water inside the tubing went downhole and the tubing went on a vacuum indicating the tubing was severed off. However the tubing was still hung in the well bore with no tubing stretch achieved when pulled on with the rig. The 5.187 production string that was in the well bore was severely rusted at surface and fell in half when the tubing was initially pulled out of the well. I consulted with my Supervisor Mike Moreland and discussed the situation and proposed plan to bring Formation Cementing in on the next working day and attempt to gain circulation on the well bore through the 2 tubing string to attempt to determine if the well bore collapsed and is bridged off or if the 5.187 casing is parted. If circulation is achieved the well will be cemented solid from the cut point of the 2.375 at 1000 to ground level elevation. In discussion with the plugging contractor Chuck Dye He confirmed the well has been idle for decades as the surface condition of the well indicated.
1517941240 2018-02-06 Plug / Plug Back No Violations Status check inspection to witness the final stages of the well plugging process accompanied on this day by ODNR Inspector Tom Thomas. On the previous day Appalachian wireline service was on location to bond logged and shoot off the 2.375 diameter 11.5 V thread tubing string used for plugging that became stuck in the well immediately after pumping the bottom hole plug. The tubing was shot off at 1,000 and Formation Cementing successfully pumped and displaced a 11.2 barrel absolute volume plug consisting of 50 sacks of class A cement with 1% gel additives. Immediately proceeding the 2.375 tubing was backed off at 300 from surface and a 6.7 barrel absolute volume surface plug set consisting of 30 sacks of class A cement with 1% gel and cotton seed hulls as additives. Upon pulling the tubing string the well bore was topped off to 30 below grade level and a permanent marker installed.
1559919184 2019-06-07 Final Restoration No Violations On this day, I am conducting a final restoration inspection. The site does not meet ORC 1509.072(B). A pit that was dug near the site for plugging purposes still remains open. Within 30 days, the site will need to be addressed in accordance with 1509.072(B): the owner or the owners agent shall remove all production and storage structures, supplies, and equipment, and any oil, salt water, and debris, and fill any remaining excavations. Within that period the owner or the owners agent shall grade or terrace and plant, seed, or sod the area disturbed where necessary to bind the soil and prevent substantial erosion and sedimentation. On 6/10/2019, I spoke with Mr. Dean Christman. He had previously been notified by ODNR Inspector Stolarik of the 6 month time frame to address the restoration.
2062030737 2018-06-19 Final Restoration No Violations Final restoration status check inspection found the well site and lease road restoration has not began. There is erosion on the lease road and well site. The tubing is still on location and has not been removed. The well was plugged on 02/06/2018. The well owner Dean Christman has been informed that the 6 month restoration timeline needs to be followed.
