1294561412 2019-06-06 Final Restoration No Violations On this day, I am conducting a final restoration inspection. The site does not meet ORC 1509.072(B). I found an area consisting of sections of steel flowline present near the lease road, and the lease road not properly restored and graded. Within 30 days, the site will need to be addressed in accordance with 1509.072(B): the owner or the owners agent shall remove all production and storage structures, supplies, and equipment, and any oil, salt water, and debris, and fill any remaining excavations. Within that period the owner or the owners agent shall grade or terrace and plant, seed, or sod the area disturbed where necessary to bind the soil and prevent substantial erosion and sedimentation. On 6/10/2019, I spoke with Mr. Dean Christman. He had previously been notified by ODNR Inspector Stolarik of the 6 month time frame to address the restoration.
1516838537 2018-01-24 Plug / Plug Back No Violations On site inspection accompanied by ODNR Supervisor Mike Moreland, also on site is a service rig owned by Chuck Dye and Formation Cementing Service. The contractor Chuck Dye was able to get the 2 Formation Cementing trucks on location but was unable to get the water truck owned by Monroe Drilling onto location due to the steep terrain and extreme mud. I requested to Mr. Dye that He erect either a plugging pit or a way to control the surface flow of well bore fluid generated to surface while plugging the well. Mr. Dye was uncooperative and indicated there was a hole in the casing and the fluid was going to run out the hole and into a nearby stream. I informed the Supervisor on site Mike Moreland that Mr. Dye was not cooperating with my request as the law describes in 1501:9-9-04 section C.
1516911990 2018-01-25 Production Wells No Violations On this day I was on site accompanied by ODNR Supervisor Mike Moreland to witness the well plugging process. The well was observed to be equipped with 6.625 diameter production casing at surface and 18 wooden conductor with the annular space between the casing and conductor open to a measured depth of 20. Formation Cementing was the plugging contractor on site and 3 plugs were set in the well bore through a 2.375 diameter tubing string. Cement returns were witnessed to surface and stayed up to surface during and after the setting of the top 300 plug.
1868021808 2018-06-19 Final Restoration No Violations Final restoration status check inspection found the well site and lease road restoration has not began. There is erosion on the lease road and well site. The tubing is still on location and has not been removed. The well was plugged on 01/25/2018. The well owner Dean Christman has been informed that the 6 month restoration timeline needs to be followed.
