1482254261 2016-12-20 Plug / Plug Back No Violations On site inspection to witness the well plugging process with a service rig on the hole owned by Chuck Dye and Formation Cementing. The well is equipped with 6.625 casing and 2 production tubing, the 2 production tubing has been pulled out on the hole- tallied and ran back in to a measured depth of 1,584 with a well bore total depth of 1,604 and a shot hole completion. A 100 sack-22.5 barrel total mix bottom plug was pumped with the well circulating production fluid to surface. The tubing was pulled up to 1,284 and a 300 -61 sack 13 barrel total barrel plug was set to isolate the bottom of the 6.250 and the top of the shot hole from 1,284 to 900 with approximately 20 barrels of oil circulating to surface along with production fluid. The tubing was then pulled up to 300 and a surface plug set with cement circulated to surface. 20 barrels of 6% gel spacer was ran between the plugs.
1495450101 2017-05-22 Final Restoration No Violations Status check inspection on the Final Restoration found the well has been plugged and the casing cut off 30 below grade and a permanent marker installed. All production related equipment has been removed. No pits were dug during the plugging process and the location along with the lease road has been rough graded in. The landowner has waived the restoration regarding the final grading and establishing vegetation.
1516386335 2018-01-19 Final Restoration No Violations A Final Restoration follow up inspection found all the production equipment removed and the final grading needing completed. On 5/22/2017, the producer indicated the landowner would sign off on the restoration. I previously assisted the producer Dean Christman with the correct documentation needed to complete the landowner waiver. There is no indication the forms were ever submitted to the Division.
