Called owner and advised him to place well back into production or file for plugging permit.

1478628303 2016-11-08 Plug / Plug Back No Violations 11/08/2016 On site inspection to witness the well plugging process with a service rig on the hole owned by Chuck Dye. Ground level elevation was 1,075 and 6.250 was in the hole to a unknown depth,. 2 tubing was set at 1,442 with TD being at 1,462. A bottom 100 sack class A plug was pumped into he shot hole through the 2 tubing set at 1,442 with 10 barrels of 6% gel space pumped ahead and the well circulating during the 1st barrel of 4 total displaced to a depth of 1,150 with the well flowing back 1/2 bbl indicating the fluid was static and cement top close to 1,150, The tubing was pulled to 1,042 and a 300 plug was set to cover the bottom of the 6.625 production -surface string with the well bore circulating constantly during the setting of the plug and being static upon ending displacement at 3 bbl of fresh water. Thetuing was then pulled to 300 and a 55 sack surface plug was set. No formation or casing information was available and the only casing string present at surface was 6.625 in diameter with the casing historically set in the area to ground level elevation, with elevation recorded by GPS at 1,075. There was a additional 39 sacks of cement pumped down the annular of the 6.625 from surface.
1942965178 2017-08-10 Final Restoration No Violations Final Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established. No plugging pit was excavated and the well was plugged on 11/08/2016.
