| 1499356777 | 2017-07-05 | Plug / Plug Back | No Violations | The purpose of my inspection was to witness plugging operations. The well was believed to be drilled to a depth of roughly 1450 feet within the Keener Sands formation. RBDMS and DC Oil and Gas do not have any records on the well. The well consisted of a wooden conductor, 6.625 inch diameter casing, 2 inch tubing broken off and crimped at 281 feet, 1 inch tubing lost downhole, and a gas escape packer at an unknown depth. The 2 inch diameter tubing, gas escape packer, and 1 inch diameter tubing were all stuck within the 6.625 inch diameter casing. Honey Run Well Services ran a spear down to 281 feet, opened up the top of the 2 inch tubing and then tripped out of hole. An overshot on the end of the 2.375 inch diameter tubing was run down to 281 feet to connect with the 2 inch diameter tubing stuck down hole. Extensive amounts of formation cave-ins have intruded into the well from holes within the 6.625 inch diameter casing. Honey Run Well Service, Appalachian Well Surveys, and Formation Cementing were on location. John Hudson with Appalachian Well Surveys rigged up to the well to run a bond log within the 2 inch diameter tubing in attempts to tag bottom and verify the cave-in depths on the backside of the 2 inch diameter tubing. The bond log verified bottom at 1128 feet and recorded cave-ins from 1128 feet to 330 feet along with multiple holes within the 2 inch diameter tubing. Honey Run tripped 1 inch diameter tubing into the 2 inch tubing to 1100 feet in depth. Appalachian Well Surveys left location and Formation Cementing rigged up to the 1 inch tubing and set a 11 sack plug of class A cement from 1100 feet to 281 feet through 1 inch tubing within the 2 inch tubing. The plug was displaced with 7.8 gallon of fresh water. Honey Run removed the 1 inch tubing from the hole and pulled one joint of the 2.375 inch diameter tubing pulling the overshot up off of the 2 inch diameter tubing starting at 281 feet. Formation cementing nippled up to the 2.375 inch tubing and pumpe |

| 1988291235 | 2017-06-28 | Plug / Plug Back | No Violations | Status check inspection on the well plugging process found a service rig on the hole owned by Chuck Dye. The well is equipped with 6.625 diameter casing with 2 diameter tubing broke off down hole at approximately 281. Information gathered from the owner Dean Christman indicated a gas escape packer was set in the well bore within 75 of TD and the well was producing from the Keener Sand at approximately 1,500. The service rig has ran a lead impression block and determined the 2 is looking up and not restricted. The rig has also been successful at grabbing ahold of the 2 that will not pull free and appears to be still connected to the gas escape packer. A mule shoe overshot will be ran over the 2 and the rig will attempt to run their wireline to bottom. If the rig can get to bottom a logging truck will be brought in and holes perforated in the 2 diameter tubing below the packer and 1 diameter tubing ran inside the 2 and the bottom hole plug will be set through the 1. The 1 will have to be pulled and the more perforations shot in the 2 diameter tubing and the 1 diameter tubing ran back in and the well plugged through the 1 diameter tubing. The 2 diameter tubing lost in the hole is not recoverable. |
