The rig has pulled the 2 tubing and rods out of the hole and ran in and tagged bottom at 1,588. The only casing present at surface is 6.625 set to a unknown depth and the well bore is a shot hole completion. The 2 diameter tubing was then ran back in the hole to a measured depth of 1,567. Formation Cementing pumped a 21.5 barrel bottom plug into the shot hole with 10 barrels of gel pumped ahead. The plug consisted of 100 sacks of class A cement and 1% calcium chloride and 50 pounds of loss circulation material. The plug was displaced to 1,250 with 3.7 barrels of fresh water. The tubing was then pulled up to 1,250 and a 11 barrel plug set from 1,250 to 950 to isolate the bottom of the 6.625 historically set to ground level elevation 1,106 plus 100 to isolate the red rock formations. In attempt to prepare to set the surface plug the well was circulated with the borehole taking 20 barrels to load and then being on a vacuum sot he 2 was pulled and the last plug was tagged with the rig wireline at 1,146and the fresh water fluid level marked at 190, the casing appears to have holes in it at the upper coal seam around 450 so a bridge was built in the well bore at 550 and the 2 tubing ran back into the well bore to 540 and 16.1 barrel class A surface plug then set from 540 to surface with the well circulating and cement returns to surface, the 2 tubing was pulled out of the hole and the well topped off to 30 below grade. The cement stayed up to surface and did not fall back. |
| 1442428974 | 2015-09-16 | Production Wells | Violations Noted | On this day I received a complaint from Phil Keevert with the Monroe County EMA who indicated His office had received information regarding pollution & contamination from a oil well tank battery off of SR 800 South of Woodsfield. Upon arrival the following equipment was noted at the well head 6.250 casing-2 production tubing and pumping rods with the production valves open and the well set up to be produced by the means of a pumping unit. At the production fluid storage area a single 100 barrel tank is present inside of a established dike area with a fluid separator present on top of the tank. Addressing the complaint, It was observed that a 1.250 discharge line was running from the fluid separator over the hill to point of EPE 20 -39.70403 X 81.09387, at this point the line was found to be open to the atmosphere with visible signs of all vegetation including trees to be stressed. Measurements were taken to a point of 207 below the impact point to a wet weather stream be where at 13:55 pm a small puddle of standing fluid was Titrate tested for chlorides with the reading exceeding a low level strip test of 611 parts per million and reading 4,300 parts per million on a high range strip test. The well owner Dean Christman was notified by phone with the following directions. Stop surface discharge and remediate the affected area. Relocate the production tank back at least 50 from the traveled portion of the roadway. The owner has been given 10 days to comply. |
