1370914816 2015-12-07 Final Restoration No Violations Final Restoration status check inspection found the well head has been cut off and the area graded with vegetation established. The well sat next to the road and no roadway was present to be restored.
1432747420 2015-05-27 Production Wells Violations Noted Status check inspection as a response to complaint # 5035. Upon inspection of the well head the following equipment was found 6.625 casing along with a 4.5 casing set as a liner and 2 tubing and pumping rods with only a tubing valve in the open position and the well set up to be produced by the means of a pumping unit. No gas sales line is present. A single 100 barrel production tank is present. RBDMS records show 8 barrels of commercial oil production since 2006. Instructions are as follows -Plug the Said well within 180 days -ODNR Monroe County Inspector Bill Stolarik 740-507-6592.
1446124393 2015-10-29 Plug / Plug Back No Violations Administrative inspection to document the plugging progress. On this day I met on site with the plugging contractor Chuck Dye who indicated they have drilled the cement bridge inside the 4.5 casing down to 525. They are bringing a larger mud pump to help circulate on the hole.
1446562912 2015-11-03 Plug / Plug Back No Violations On site inspection to witness the well plugging process in response to complaint # 5035 and violation # 1432747420 issued on 05/27/2015. On this day JET Wireline ran a cement bond log showing the top of cement on the 4.5 casing string that was set and cemented on 08/04/2008 to a depth of 1,413 with Formation Cementing on location to cement the casing using 50 sacks of class A cement which would be a estimated fill up of 500 between the 6.250 and 4.5 . The cement bond log showed the top of cement at 938 and JET wireline shot the casing off at the 1st free point of 762. A service rig was on location owned by Chuck Dye and then pulled 2 joints of 4.5 and bailed the well dry. Christman Supply the grouted the well with 2 cubic yards of 9 bag grout mix from surface, the 4.5 was then pulled from the hole, top of cement will be tagged the next day and any fluid bailed off before finishing grouting the well to surface.
1446729412 2015-11-04 Plug / Plug Back No Violations 11/04/2015 the cement top was tagged at 550 inside the 6.250 and no fluid was confirmed present, the 6.250 was the grouted from surface with 4 cu yd of 9 bag grout mix from Christman Suply. The well bore has now been filled solid from TD to surface with cement. The previous violation # 1432747420 and complaint # 5035 is considered resolved at this time.
1723746320 2015-09-11 Plug / Plug Back No Violations I am onsite to witness the bottom plug placement. I witnessed 5.5 yards of nine sack grout being placed into the well in an attempt to fill the shot hole. The well bridged off forty feet from surface after placing 5.5 yards. The bridge will be drilled out and the cement top located before proceeding further.
1831140811 2015-10-26 Plug / Plug Back No Violations On site status check inspection follow up on plugging process. The producer initially attempted to grout the well from surface and experienced a downhole casing bridge while conducting the process. Inspector Eric Conrad was on site to witness the process and reported approximately 5 cubic yards were into the well bore before the bridge occurred. A service rig is on site owned by Chuck Dye and is currently drilling out cement at a depth of 300. The owner Dean Christman reports there was approximately 1,400 of 4.5 casing set in the well on a packer and cemented in.
1963649251 2015-10-21 Plug / Plug Back No Violations My inspection today finds the plugging contractor, Honey Run Oil and Gas, is utilizing a power swivel to drill out a cement bridge in the 4.5 casing. They have drilled 45 of cement out of the well.
