0209706057 2019-02-19 Production Wells No Violations On this day, I am conducting a follow-up inspection in reference to Complaint #5775. I found the well equipped with casing and tubing with rods inserted into the tubing. Steel flowlines are attached, but no pumping unit is present. The well is surrounded by thick vegetation, limiting access to the lines, and does not indicate signs of recent activity. No identification is present. Multiple pictures were taken.
1145349687 2018-01-23 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik to conduct a follow-up inspection to complaint 5775 filed by the landowner. Please reference his comments for a detailed report.
1454946344 2016-02-08 Production Wells No Violations Status check inspection found the following equipment present at the well head-6.625 casing -4.875 production casing - 2 production tubing and pumping rods with both the tubing and casing equipped with 2 flow lines with no valves present on either flow line, the well is set up to be produced by the means of a pumping unit with an electric service present at the well head but no sign of recent activity around the well. No production tank was identified as related to the said well. Historically a local operator identified as Chuck Dye has claimed to own the lease but is unregistered as an owner with the Division and the well is not registered under any operators Bond. The Division was advised that the well is present and not plugged.
1478430717 2018-01-23 Production Wells No Violations On site inspection as a response to complaint 5775 filed by the landowner indicating the individual (Chuck Dye) claiming to own and operate the lease and has failed to pay any royalties to the property owner during the time frame the landowner has been in possession of the property which has been (8) years. The landowner also indicated the wells were idle and He was not using or receiving domestic house gas from any of the 3 wells on the lease. The well was observed to be equipped with 6.625 diameter production casing and 2diameter production tubing with pumping rods inserted into the production tubing but no pumping unit present attached to the well head. No flow lines appear to be attached to the well head. The well head was engulfed in dense vegetation limiting visibility. No registered owner is showing up in the State RBDMS database and no reported production is on file.
