| 1362942283 | 2019-05-09 | Production Wells | No Violations | Status check inspection accompanied by the well owner Dean Christman found the well standing open with 7 casing only present at surface. Mr. Christman indicated the 7 was attached just below surface to the original 6.250, the 6.625 was damaged. It was also indicated that in the year 2010 the produced set a 100 sack bottom hole plug while attempting to deepen the well. It was indicated the attempt to deepen the well was unsuccessful due to metal in the well bore. The well owner did have a copy of a cement ticket for the bottom hole plug. The well owner Dean Christman will bring his rig in and run tubing into the well bore to determine a total depth. The well will then be plugged from there to surface accordingly. A plug permit has been issued. |
| 1558527632 | 2019-05-22 | Plug / Plug Back | No Violations | I am onsite to witness the plugging operation conducted by Dean Christman and Formation Cementing. Dean Christman has a service rig on the hole and Monroe Drilling has (1) water-truck onsite. An earthen pit was dug with a plastic liner for fluid collection. Formation Cementing has a pump truck and bulk truck on location. Additionally, Appalachian Well Surveys ran a GR/CBL/VDL/CCL log on 5/21/2019, which indicated the bottom of surface casing to be set at a depth of 1,139 feet. Mr. Christman inserted 1.9 inch diameter tubing and tagged bottom at 1,613 feet. The tubing was then set at 1,605 feet, circulation was established, and Formation Cementing pumped the Keener Sand/ Bottom of Surface plug (1605 - 1039) with 90 sacks of Class A cement with a 1.18 yield and weight of 15.6 pounds per gallon. It was displaced with 2.6 barrels of fresh water. It will be noted that the Keener Sand interval is unknown due to lack of records. Then, they pumped the surface plug that consisted of 65 sacks of cement with a 1.18 yield, and a weight of 15.6 pounds per gallon. Good cement returns were witnessed at surface, and stabilized. |
