Upon inspection of the well head the well was observed to be equipped with 6.625 diameter casing and 2 diameter tubing with no production flow lines hooked to the tubing or casing head. The well is set up to be produced by the means of a gasoline powered pumping unit. The landowner indicated he has not witnessed the producer on the property in approximately 9 months. At the production fluid storage facility 2 steel tanks are present that a shared between 3 wells with one well being on a adjoining property and identified as 3411164034-0000, the tanks are not of common dimensional size and appear to be capable of holding approximately 80 and 50 barrels of product. There is a gravity fluid separator on top of the 80 barrel tank with a discharge line leading to a defined ditch line where product from the fluid separator may be dispersed onto the earths surface. The landowner Mike Gardner has indicated he does not wish for the well bore product to be dispersed onto his property. The landowner also indicated he has not received royalty payment in 15 years. There is also a steel production tank of approximate size of 80 barrels that is leaking at a location of Epe 9-39.75679 x 81.27142. The producer has not reported commercial production to the Division for any of the 2 wells on the lease 3411162117-0000 or 3411162118-0000 since 2013. |

The well is equipped with a 6.625 diameter production casing with a casing well head installed on the 6.625 casing and 2 diameter production tubing inserted into the casing. A 2 tubing head is present with pumping rods inserted into the tubing. No casing production flow line is attached and a 1 flow line is attached to the tubing well head with no production valve present. In the area of the well was found the production facility consisting of (2) steel design production tanks not of common dimensional size. Sitting atop the larger of the two tanks is a gravity fluid separator with a 3 plastic corrugated discharge line that has been installed from the separator and into the earths surface inside the dike and leads to a discharge point down elevation from the production facility where a 3 corrugated line was observed and photographed to be present and open ended. There is dense vegetation at both the production facility and the well head as in my previous inspections. No sign of recent activity is present at the well head or production facility. Production has been reported up until 2016. |

On this day it was observed that the issues previously indicated on the initial violations concerning pollution and contamination along with well identification and proper diking around the production facility have been addressed at some point in the previous years and I consider those issues addressed. Also noted is this is my fourth trip to the well since 03/29/2017 and the condition of the well contradicts the production being reported by the producer. The well is equipped with 6.625 diameter production casing and 2 diameter non upset tubing with tubing and casing well heads present. The tubing head has no valve and the flow line is disconnected. The casing head has a valve and the valve is open however the no flow line is present on the casing head either. The well is set up to be produced by the means of a pumping unit with an gasoline driven engine. There is dense vegetation present around the well head and pumping unit and no sign of recent human activity in the area. Near the well head is the production fluid facility that consists of 2 steel construction production tanks with well owner identification present. The tanks are situated inside an earthen dike. Dense vegetation is present at the production facility restricting access and visibility to determine the tank sizes. The landowner has also filed a complaint identified as # 5968 in the complaint log database. |

The initial complaint indicated there were (2) wells on the landowners property in Franklin Township Monroe County identified as the Hogue Mason # 1 and # 2 well. The landowner also indicated a well on a neighboring property that is situated in Summit Township of Monroe County is also producing into the production facility on his property without his consent. The landowners initial concerns were directed to the question of how proper production was being recorded and reported when wells on 2 separate leases were produced into the same common tanks. Also indicated in the complaint was that production water from both tanks was being discharged onto his property and he wished the discharge to cease. I have visited the lease 5 times since 03/29/2017 and have found no indication that the wells have been produced from that date to current day. The wells have became increasingly engulfed in vegetation since my first visit. The said well is equipped with 6.625 production casing and 2.375 diameter production tubing with both casing and tubing well heads present. The casing head is equipped with a 2 diameter valve attached to the well head, the valve is in the closed position. The tubing head has no valve present and the flow line is disconnected at the well head. The well is set up to be produced by the means of a gasoline powered pumping unit. The production facility consists of 2 steel tanks that are not of common size and appear to be 50 and 80 barrel approximated size capacity. Both tanks are equipped with gravity fluid separators with 3 corrugated discharges that are constructed to allow product to discharge inside the dike area. There is a earthen dike constructed around the production tanks and ID posted on the tank battery only.
