0411623379 2015-10-28 Production Wells No Violations Status check inspection in response to complaint # 5160 received on 10/27/2015 from the landowner Schawn Seitz who indicated there was product spilled on the ground when a tank was removed. On this day 10/28/2015- I inspected the general area indicated in the initial complaint to have oil product spilled on the ground to find no hydrocarbon product present in the form of raw product, a sheen, or paraffin. The area was walked on foot and reviewed extensively. It was observed that the producer was installing a new production facility to the effect of a new 100 barrel capacity steel tank and a 1,600 gallon capacity plastic tank to be used for brine, all the production equipment was being placed inside a dike area. The alleged owner of Genesis Resources a one Andrew Allpin was contacted on 10/27/15 and Mr. Allpin indicated he would comply with any remedial clean up directions but was unaware of any product present or discharged during the recent tank removal. I contacted Mr. Allpin once again on 10/28/2015 at 10:58 am and informed Him that my inspection found no product present warranting clean up. No violations were noted on the day of my inspection and the complaint is considered addressed.
1103785804 2013-08-21 Production Wells No Violations Follow up inspection from a call received from Jenny Davidson with the US EPAs Chicago District 5 office. Upon inspection it was observed the access road gates are locked and the road blocked with a trailer denying the producer access to the property. The 100 barrel production tank was checked and found empty with no signs of product outside the tank. At the well head it was observed that the well has not been in current fluid production and did have a gas line hooked up with all valves open at the well head, in conversation with the producer Ed Byers He stated that once the legal issues between Himself and the current lease holders are resolved He will make the proper correction to the production tank dike and put the well back into commercial production. Multiple pictures were taken and the area checked with no contamination found.
1151508896 2010-05-14 Production Wells No Violations Per Rob Stonerock I went in to find out info on pump jack. Such as size and amount of weights. The were no.466 on the gear box. There was 12 plates that measured 21 3/8x19x1thick.The polished rod was 1 1/4 thick. The jack measured 30wide x 62tall.The walking beam is 100long x 8high x 5 1/4 wide. There are counter weight on the base of the pump jack. The back of the jack is anchored down.
1267629781 2010-03-01 Production Wells Violations Noted The day of inspection I met with Shawn Seitz on the location where the said well is pumping and brine water is coming out of the separator going down creek that feeds an intermittent stream. The well is not registered. There is not an id on well or tank. Byers claims that the well makes gas for the house. The landowner said that the well makes three hundred barrel of oil a year. There might be a total of two wells Immediately cease discharging of brine in the creek. Discharge water into oil tank or a plastic tank until the weather warrants moving equipment in to work on this site. Move tank and place an id at wellhead and or the rancid must include:owner,county,permit no.,twp.,emergency contact no.
1270493265 2010-03-30 Production Wells No Violations Rob Stonerock and I went in to the site. Location was wet. Byers had started diverting brine into tank battery. Byers had called and I told him that either haul the brine or bring a service rig in and pull the well because the brine is at 86,000 ppm in the chlorides.Barium is also high.No id or bond as of this day of inspection.

1320287737 2013-08-20 Production Wells No Violations On this day I received a call from the Jenny Davidson with the US EPAs Chicago office, She stated that Shawn Seitz has called every day for the last 2 weeks, I briefed Her on the situation and agreed to reply to Her office through E-Mail, I contacted my supervisor Rocky King and Division Public Information officer Jocelyn Kozlowski and informed them of the situation. I shall do a follow up inspection on the well.

1355444044 2012-12-13 Production Wells No Violations Inspection and meeting on this day per request of producer Ed Byers at the well identified as the O.S Marley # 3. Division Supervisor Rocky King and Inspector Bill Stolarik met with MR. Byers to discuss the current status of the prior violations and the Ohio Revised Code laws regarding the production of oil & gas. Mr. King explained to Mr. Byers that He has met the demands of the prior violations and could resume production under the understanding that He continue to operate the well in a environmentally friendly manner and continue to report the annual production to the division. Discussion in regards to the production storage facility location and proper construction according to SPCC rules was discussed and agreed upon by both parties. It was explained to Mr. Byers that any legal matters between the landowner and Himself was a private matter not to involve the Division. Inspector will note that the well was purchased and is produced by Mr. Byers for the primary purpose of supplying domestic house gas to the Byers residence which is located on the adjoining property that the well is located on.
1355841529 2012-12-18 Administrative Inspection No Violations Inspector received a call from Shawn Seitz in regards to the recent inspection and meeting on 12/13/12 with Ed Byers- supervisor Rocky King and inspector Bill Stolarik concerning the O.S Marley #3 well. Mr. Seitz was advised that Mr. Byers had met the requirements of the previous violations and could resume production as long as he did so in a environmentally friendly manner. Mr. Seitz was argumentative and contested the fact that the soil and water samples were not thorough and incorrect. Inspector Stolarik advised Mr. Seitz that the State had found no grounds to pursue this matter and the current issues were considered resolved and any legal matters were between Him and the producer not to involve the division. Inspector will note that a prior complaint on this issue by the complaint number of 3853 had been resolved by Division Hydrologist Kyle Tharp.
