0875993428 2014-05-21 Production Wells Violations Noted Status check inspection found the well standing open with 6.625 casing present with no production tubing valves or flow lines and the well incapable of production in its current state. There is a work over pit over the bank that was not closed and 4.5 casing and 2.375 tubing and various equipment and trash scattered over the location, RBDMS records show no reported production on record. Instructions are as follows plug the said well within 180 days, ODNR contact - Monroe Co Inspector Bill Stolarik 740-507-6592.
1362448257 2014-09-30 Administrative Inspection No Violations Administrative inspection to document the phone call received by the well owner Jim Yontz at 9:21 AM on this day I was contacted by Jim Yontz in regards to the said well and violation issued against the well, Jim was given the contact information for the Division to request a plugging permit.
1429037977 2015-04-14 Production Wells No Violations Compliance Notice # 875993428 issued on 05/21/2014 and expired on 11/21/2104 follow up inspection. On the day of my inspection it was observed that the only casing present was 6.625 and it was standing open with no gas smell present or no fluid level noticeable. I shall contact the producer in regards to the active unresolved violation and idle well.
1430392348 2015-04-30 Administrative Inspection No Violations The initial violation # 875993428 issued on 05/21/2014 has expired on 11/21/2014. All documents have been prepared and forwarded over to the Regional Supervisor for review and Order referral. A plugging permit has been received but not acted upon.

1464104522 2016-05-24 Plug / Plug Back No Violations On site inspection to witness the well plugging process. Initially the well owner indicated there was 70 of lost tubing in the hole at a depth of 1,332. Wireline was ran in the hole off the service rig and the depth confirmed at 1,332, the rig then ran 2.375 tubing in the hole, first tagging the lost tubing and then setting the tubing at 1,330. Our initial attempt to establish circulation failed with the well on vacuum. Historic records show this as a Keener well however the operator reports the well was producing from the lime section, Formation Cementing first pumped a cement 50 sack plug followed by a 400 6% gel spacer with the well on vacuum after displacement. We then let the cement set for 1 hour and pumped a 400 6% gel space ahead of the 2nd plug which consisted of 70 sacks of cement with 2% calcium and one 40 lb bag of cotton seed hulls with the cement company seeing 150 lbs. lift pressure on the pump. All fluid had been pumped out of the 2.375 tubing set at 1,330with the tubing being pulled after the 2nd plug and the cement allowed to set and cure for 2 hours before attempting to tag the top of cement, fluid level was seen on the tubing at 400 when pulled from the hole. The tubing was ran back in to 1,300 and a 60 sack 400 plug was set to cover the bottom of the 6.625 which was make on a previous log at 1,210. The 2.375 tubing was pulled up to 200 and a surface plug set. All previous violations resolved on this day.
