1414155649 2014-10-24 Plug / Plug Back No Violations On the day of my inspection I was on site to witness the plugging of the said well with Howells Oil & Gas company owned service rig on location along with Formation Cementing Service. A bottom hole total depth was confirmed at 1,480 and 2 tubing ran into 1,475 with a 300 bottom plug of 47 sacks of class A cement set inside a 6 open hole and displaced with 4.7 bbl of fresh water and allowed to set 1.5 hrs before tagging, with the1st plug tagged @ 1,250 and the 2nd plug set from 1,250 to 950. A 3rd plug to cover bottom of surface estimated @ 600 set from 830 to 500 then the surface plug set from 400 to ground level with good cement to surface and the plug staying up to surface.
1431536097 2015-05-13 Final Restoration No Violations Final Restoration status check inspection found the well site and affected area meets ORC 1509.072 with vegetation established in all affected areas and all equipment removed.
