1470069542 2016-08-01 Production Wells Violations Noted On site status check inspection found the following equipment present at the well head-6.625 production casing -5.187 casing liner-2 production tubing and pumping rods with a production tree on the 2 tubing and the tubing and casing production valves in the open position and the well set up to be produced by the means of a pumping unit. At the production fluid storage area a single 60 barrel production tank is present inside a earthen dike containment area. No identification is posted and the fluid inside the containment dike has a red cast stain discoloration and noticeable odor consistent of brine water with approximately 10 barrels of standing fluid present. The fluid was titrate tested for chlorides with both high and low Quantab strip test finding chloride levels exceeding the low level chloride range at 644 parts per million and also exceeding the high level chloride range at 6,171 parts per million chlorides. Also noted no production has been reported to the Division since 2010. Instructions are as follows Immediately recover any standing fluid from within the dike area and case any discharge activity, report production to the Division for the time period the said owner has been in possession of the well. Post proper well identification. Complete all tasks within 15 days.
1496132670 2016-08-02 Production Wells No Violations On this day I visited the well site being accompanied by ODNR Supervisor Brent Bear. The inspection purpose was to follow up on violation # 1470069542 issued on the previous day 08/01/2016. Upon inspection it was observed that the brine fluid I observed inside the containment dike on 08/01/2016 was not present. The product was tested for chlorides and photographed on the previous day 08/01/2016, I also spoke with a Jason Ludwig on that evening, Mr. Ludwig indicated he had indeed drained the brine from the production tank into the dike area. Also noted that on this day supervisor Brent Bear and myself had a phone conversation with the Divisions Bonding and Surety Supervisor Jon Austin who had followed up on information I provided him the previous day regarding the well ownership and the well being placed under the wrong owners Bond. Mr. Austin indicated he spoke with a Jason Ludwig with PP&G and was informed that indeed PP&G was the owner operating the well. Mr. Austin indicated they would peruse the matter and see the well registration is corrected. I shall request the brine hauling records from PP&G.
2105909667 2018-12-10 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. I found the well equipped with 6.625 inch diameter casing, 5 inch diameter casing liner, and 2 inch diameter tubing with the production valve open. It is setup to be produced by means of a pumping unit with rods and an electric motor. The production facility consists of (1) 60 barrel steel tank within an earthen dike. The production records, reported by PP & G Oil, are current through 2017, with zero (0) reported for 2015-2017. No identification is present at the tank or wellhead. I have previously spoke with Rick Kroll, of Ludlow Oil and Gas (owner #4938), and he disputes ownership of the well. I spoke with Derrick Ludwig on 12/10/2018 and he indicated that PP & G Oil operated the well, so I requested identification be placed at the wellhead and/or tank. Ownership will need adjusted. Additionally, there is no sign of pollution and contamination at the wellhead or in the dike area, as previously noted. Pictures were taken.
