Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1323268911 2011-12-05 Production Wells No Violations On day of inspection well was complete with pump jack. Tank batter contents included: one 110 bbl oil storage, 50 bbl brine tank, and a gravity separator. No violations noted.
1361815341 2013-02-25 Production Wells Violations Noted Inspection of the tank battery found a 100 barrel tank with a 55gallon oil-water knock out separator on top and a 50 barrel tank all located in a enclosed dike that has obvious signs of pollution and contamination in the bottom of the dike from where the knock out separator has backed up and overflowed. Instructions are as follows Clean up all oil and contaminated soils removing soil to an EPA approved landfill; stop dispersant of production waste water outside of the tank battery and have the production waste water hauled to a disposal well. Post proper legible identification. Properly register all producing wells on the lease with the Division.
1362419640 2013-03-04 Production Wells No Violations Compliance agreement follow up inspection found the pollution and contamination inside the tank battery has been cleaned up and the dike area remediated. Inspection of the well head found the following equipment present-6.625 casing -2 tubing and pumping rods with a .750 vent packer string with the well only capable of supplying gas at the time of my inspection due to the means of motive power missing from the pumping unit and the tubing valve closed. A 100 barrel production tank is present in the tank battery. Inspector did receive notification from Rob Armstrong this morning stating that the oil has been cleaned up, Rob also stated that the well did not belong to Him that it belonged to His son. I informed Rob Armstrong that the Database indicated He was the owner as well as the identification on the tank battery and that He would have to speak with the Divisions Registration Dept.
