0099883119 2015-10-01 Production Wells No Violations Compliance Notice # -1245345315 follow up inspection. On this day I observed a service rig on location owned by Chuck Dye. The rig operator has reported the total 2 tubing recovered initially was 1,599 to the packer. A initial 160 has been recovered that was below the packer. There is still 2 tubing in the well and the rig operator has ran in with a overshot fishing tool and grabbed ahold of the tubing and shall pull tension and allow the tubing to sit overnight to attempt to let the tubing relieve itself down hole.
0106514828 2016-07-14 Production Wells No Violations Compliance notice # -124534315 issued on 03/16/2015 follow up inspection found the following equipment present at the well head-6.625 production casing -2 production tubing with the tubing head not connected to the 6.625 casing and the hole previously present in the 6.625 at grade level not addressed. A production valve is attached to the 2 tubing however no production flow line is present attached to the tubing. There was a pit area below the well head that was excavated during the initial oil spill event that has standing well bore product in it that needs to be recovered and removed from the location. Also the area disturbed during the well repair has not been repaired and vegetation has not been attempted to be established.

Upon inspection it was observed that the producer has made corrections to the well head and well bore to allow the well to be capable of production and not to leak well bore product onto the earths surface. There is a 2 flow line attached to the 2 tubing at the well head.
0359426434 2015-09-30 Administrative Inspection No Violations Administrative inspection to document the conversation between the contractor Chuck Dye and myself at 8:48 am concerning the current service work being done on the well. Mr. Dye informed me he was on location and was attempting to recover the tail joint of 2 tubing that was lost in the hole. The joint was situated in the string below the packer. Mr. Dye has ahold of the tubing joint with a overshot fishing recovery tool and is attempting to recover it. Mr. Dye will contact me later in the day with an update.

Upon inspection it was observed that a service owned by Chuck Dye rig is rigged up on the hole and is currently running in the hole with 2375 tubing with a 2 tubing overshot attached to the bottom, the rig will attempt to get over and attached to the 2 tubing that is currently in the wellbore bottom at a depth of 1,901, currently to date 300 of 2 tubing has been recovered that was lost in the hole below a formation packer that was set at 1,599.

The producer Scott Cisler was contacted on this day by phone, Mr. .Cisler indicated the a local contractor by the name of Chuck Dye was scheduled to move his service rig in and begin repairing the well on 09/21/2015.

1245345314 2015-03-15 Production Wells Violations Noted On site inspection to witness the clean up from the recent oil spill found the well owner on location conducting a on site clean up and recovery with 3 men on location and 2 pieces of equipment. Several containment Oil soak booms and dikes were constructed in the affected wet weather stream bed next to the well head. The material affected by the discharge has been stockpiled on location until weather conditions allow for removal. The Ohio EPA has been involved and on site. It has been determined that the well bore allowed fluid to migrate up the backside of the un-cemented 7 surface casing from a shallow producing zone initially discovered on or about 03/10/15 and reported to local authorities but not the Division until the day of 03/14/15. It was also observed that the 7 casing has a large hole in it near the surface. The well was found also to be idle commercially with no reported production in the database. Instructions are as follows -Plug the said well within 180 days. ODNR Monroe County Inspector Bill Stolarik- 740-07-692.

1262495633 2016-03-29 Preliminary Restoration No Violations Compliance Notice # -1245345315 follow up inspection found the following present at the well head 6.625 casing and 2 production tubing with a 2 brass ball valve on the tubing in the closed position and no production flow line present. There is oil on the ground near the well head that will need cleaned up and the area disturbed recently when a service rig was brought in will need restored with erosion present on the lease road. Also noted was the recent rig work done to the well in the effect of removing old production tubing and isolation packers and the setting of a hook wall packer on new 2.375 tubing. There is no proper surface control on the 6.250 casing and the holes in the 6.250 have not been repaired.

1427041505 2015-03-20 Production Wells No Violations Status check Compliance Notice follow up inspection in regards to the recent oil spill reported to the Division on 03/14/15. Upon inspection on this day it was observed that the stockpiled soil contaminated with oil has been removed from the well site and placed in a container along the County Road for pick up and removal to a EPA approved landfill. The affected waters of the State are still being monitored with spill recovery oil absorbent booms and 2 x 2 oil soak rags.
1442948504 2015-09-22 Production Wells No Violations Compliance Notice # -1245345315 follow up inspection. On this day I witnessed a service rig on location owned by Chuck Dye, they were attempting to pull the 2 tubing string out of the hole, the tubing was hung and the rig pulled approximately 14 of stretch. They attempted to work the string free unsuccessfully. After approximately 1 hour stretch was pulled tight on the string and the rig brake locked down at that point to allow the downhole packer to possibly relieve itself overnight.
1443032653 2015-09-23 Production Wells No Violations Compliance Notice # -1245345315 follow up inspection. On this day I witnessed a service rig on location owned by Chuck Dye. The rig had recovered approximately 1,300 of 2 tubing from the well bore, there was a old style rubber seal compression set down packer attached to the tubing on bottom. The measured total depth by wireline was 1,584. I spoke with Chuck Dye who indicated a electronic wireline truck was being brought in to find the depth of the 6.250 casing.
1447874292 2015-11-18 Production Wells No Violations Compliance Notice # -1245345315 follow up inspection. On this day I witnessed JET Wireline service on location and ran a Gamma Ray -CCL log with a well bore measured depth of 1,958. The 6.250 casing had holes in it between the depths of 600 to 610,832 to 854 , 940 to 1010, the Keener Sand was marked at 1,458 to 1,474 and the Injun was marked at 1,478 to 1,524. Moving foreword the service rig on location owner and project coordinator Chuck Dye indicated they had ordered a Pharmaco packer that will be set in the well bore open hole section below the 6.250 which the log showed to be set at 1,214. Ground level elevation by GPS is 1,030.
1448538261 2015-11-26 Production Wells No Violations 11/25/2015 - 2.375 has now been set with a formation packer at 1,900 and 74 of 2.375 tail below the packer. Top perforation in 2.375 was at the top of the Berea -1,961.95. The service rig owned by chuck dye is currently swabbing the well into the 100 barrel production tank with the fluid level at 200 from TD and the well cleaning up.
1467059038 2015-06-04 Administrative Inspection No Violations On 6/4/2015, I met with Mr. Scott Cisler dba Cisler Oil & Gas to discuss the status of the wells on the Sloan Lease located in Monroe County that were issued Compliance Notices. Mr. Cisler provided a proposal plan for the wells on this lease to achieve compliance. We discussed his proposed plan and Mr. Cisler was instructed that no extension would be granted on the Sloan #5 Well that had released oil into a creek. Furthermore, there were no extensions granted on this day for these wells. Mr. Cisler was instructed to continue to work towards achieving compliance. I recommended to Mr. Cisler that he continue to communicate with the inspector for the Division and speak with the landowner about his plans for these wells.

1470240150 2016-08-02 Production Wells No Violations On this day I visited the well site with ODNR Supervisor Brent Bear with the following observed -The producer has addressed the pollution & contamination and the well site has vegetation established, water brakes have been added to the lease road and a steel line has been inserted down over the 6.625 casing and the void between the 2 strings filled with cement to avoid surface water from infiltrating the pre existing holes in the 6.625 at grade level. The producer still needs to secure the well head to the 6.625 before violation # -124534315 may be considered resolved.
1513344135 2017-12-15 Production Wells No Violations Order 2017-101 follow up inspection found the well equipped with 7 casing and 2 production tubing with a 2 diameter flow line attached to the well head with a brass ball valve present in the closed position. Proper well identification is posted and photographs were taken on this day. I also spoke with the well owner Scott Cisler who indicated He was financially unable to afford insurance.

1686711991 2015-03-25 Production Wells No Violations Compliance notice # 1245345315 follow up inspection on this day with ODNR Regional supervisor Brent Bear also in attendance. On this day it was observed that the soil affected by the recent well head oil leak has been recovered and removed off site. The nearby wet weather stream bed still has filter sock booms and spill rags in place with straw bales being used as a water deterrent. The production casing was observed to have a hole in it at the ground level. Discussion took place on how to handle the issue with the hole in the casing and the well needing plugged to avoid future fluid loss into the waters of the State. A temporary fluid recovery pit has been erected below the well head to gather any material that migrate from the well bore until the issue can be resolved.
1703383190 2015-03-18 Production Wells No Violations Compliance Action # -1245345315 issued on 03/16/2015 follow up inspection. On this day I was accompanied by Inspectors from the US Forest Service (Steve Sapio) and US BLM (Jim Woods) to review the recent contamination caused from the idle well omitting hydrocarbon product onto the Earths surface and into a nearby waterway. Upon our inspection we reviewed the GPS coordinates and confirmed the owners identification as incorrect and that I had properly identified the well matching the database records and information. The area affected by the spill was reviewed and confirmed the owner is working to recover any remaining product and protecting the waters of the State. 4 other wells were visited and pointed out to the Gentlemen on this day, all residing on the Forest Service property owned by Cisler Oil & Gas and non producing with violations issued against them on 03/15/2015.
1773533158 2015-04-14 Production Wells No Violations Compliance notice # 1245345315 follow up inspection found the producer has addressed the issues with the pollution and contamination but has yet to address the issues with the well 6.625 casing in regards to the hole at surface level and that petroleum migrated annularly up the backside of the casing. The producer has been given the option to run a casing string with proper annular space and cement it to surface or plug the said well.
1974234864 2015-04-22 Production Wells No Violations Compliance Notice # -1245345315 follow up inspection. On this day I Re-Visited the well site along with Aaron Wolfe from the Ohio EPA who was the initial EPA responding agent. At the well site it was observed that the product initially released from the well head has been recovered and both the EPA and myself fell the clean up is sufficient. The well owner Scott Cisler has been in contact with myself in regards to the issue with the defective well casing, the owner has been informed that due to multiple holes in the 6.625 string that I was giving the produce 2 options to remedy the issue (1) plug the said well with cement (2) set 4.5 casing in the well bore to a point below the 6.625and cement to surface. The owner Scott Cisler has requested any information concerning the well completion we may have. My Supervisor Brent Bear has been informed of the options given the producer.

On this day it was observed that the well is still equipped with 7 casing only and no attempt has been made to isolate the 7 casing annular space. I have contacted the operator on several occasions with directions to either set a production string in the well and cement it back to surface to isolate the un cemented surface casing from the production formations or plug the well. My supervisor Brent Bear is aware of the directions given and the situation.
