There is still 2 tubing in the well and the rig operator has ran in with a overshot fishing tool and grabbed ahold of the tubing and shall pull tension and allow the tubing to sit overnight to attempt to let the tubing relieve itself down hole. |

Several containment Oil soak booms and dikes were constructed in the affected wet weather stream bed next to the well head. The material affected by the discharge has been stockpiled on location until weather conditions allow for removal. The Ohio EPA has been involved and on site. It has been determined that the well bore allowed fluid to migrate up the backside of the un-cemented 7 surface casing from a shallow producing zone initially discovered on or about 03/10/15 and reported to local authorities but not the Division until the day of 03/14/15. It was also observed that the 7 casing has a large hole in it near the surface. The well was found also to be idle commercially with no reported production in the database. Instructions are as follows -Plug the said well within 180 days. ODNR Monroe County Inspector Bill Stolarik- 740-07-692. |

Ground level elevation by GPS is 1,030. |

The owner Scott Cisler has requested any information concerning the well completion we may have. My Supervisor Brent Bear has been informed of the options given the producer. |
