0560835452 2012-11-19 Administrative Inspection No Violations Inspector received a call from Glenn Ludwig in regards to the current violations on the Jones lease, Mr. Ludwig asked if He was allowed to sell the lease to Northwood energy? Inspector informed Him that Division HQ would have to approve the transaction and that an environmental issues or charges of neglect would remain issued to Him. Discussion in regards to what is the correct permit number of one of the well did take place with the inspector informing Mr. Ludwig that a discrepency between what He considers the well permit number to be and what the division called it could be sorted out, Inspector commented that Mr. Ludwid needed to get His wells registered and that no Mississippian exemption clause made these wells exempt from proper registration and following baseline environmental standards.
0645567477 2012-10-25 Production Wells Violations Noted Upon inspection of the said well on this day found the well was pumping and to be dispersing production waste water into the said waterways of the State, In which is a violation of 1509.22, Which reads Brine from Exempt Mississippian wells shall not be discharged directly into the said waterways of the State. Failure to immediately comply with the inspectors request to shut in and stop commercial production of the said well until violations are addressed and the well is equipped in a mannerism that the production waste water may be collected and hauled to a proper disposal well will lead to immediate action taken. In which will lead the producer subject to criminal action for being in violation of 1509.22. Requests are as follows-Register the said well with the division- Stop dispersant of production fluids into the said waterways of the state- Properly Identify& construct dike impoundments around all production storage tanks taking into advisement the previous request by the inspector to set the 3-100 barrel tanks back off the public right of way- File annual statement of production with the division. Clean up and remove any and all contaminated soils. Establish proper vegetation on any affected or disturbed areas.
1287051094 2013-04-24 Production Wells No Violations Compliance agreement follow up found that the company owning the well PP&G Oil has set a secondary recovery tank and no signs of current pollution and contamination was found as in previous inspections. The company has registered the said well under their current bond with the division. Compliance notice issued on 11/02/2012was number 65567477.
1342557807 2012-11-16 Production Wells No Violations Inspection of the well found the well still in production and no terms of the compliance agreement have been met by the producer.
1455613510 2012-08-27 Production Wells No Violations I met with MRI 2 Bill Stolarik to aid and witness the collection of samples. On this day the well was producing and production water was being dispersed into waters of the state. This well is sharing a tank with well #34-111-21327-00-0. There was also oil field contamination around the well head and below the point of dispersal. Violations were noted and owner was notified via Compliance Notice.
1456482319 2016-02-25 Production Wells No Violations Administrative report to document the current workover being done on the said well. A local contractor by the name of Chuck Dye has a rig on the hole and JET Wireline service ran a Gamma Ray-CCL log on the well. The log shows a 4.5 casing liner set in the well bore on a packer to a measured depth from surface of 1,351, 6.250 is also present to a depth of 1,391. In conversation with the service rig owner Chuck Dye it was reported that He will attempt to recover the 4.5 and packer and re set the isolation string below the 6.250 shoe in attempt to isolate the large amount of water influx into the well bore. A total well bore depth was also marked at 1,651 and a copy of the log was received.
1472749070 2012-12-10 Production Wells No Violations Inspection of the well site found the well still in production and dispersing production waste fluid onto the ground surface with previous violation still pending and prior request being made by the inspector asking that the well be shut in and not produced until the well is registered and capable of being produced without further contamination. A attempt to dike around one of the production tanks was made with only 2 sides of the tank being diked making it an incomplete enclosure incapable of holding the required amount of fluid in the amount of a spill. The said well has not been registered. Inspector will note that I spoke with Jason Ludwig on 12/05/12 at another well location and during our conversation I did make note that the wells were to be registered and provided Him with a copy of the Mississippian exemption laws.
