0597699097 2018-06-18 Production Wells No Violations I was on location today to follow-up on a compliance notice issued on this well for being idle and incapable of production issued on 06/06/2017. A this time there has been no change in the status of the well. The production valves at the wellhead are still shut and the well is covered in vegetation. I contacted Dave Bonar today when I got back into phone service. Mr. Bonar stated that he is planning on getting the well back into production. It is on his list to work on.
1063082740 2016-12-06 Production Wells Violations Noted The inspection today revealed that this well is a pumping unit equipped with a 4 1/2 inch diameter wellhead, 2 inch diameter steel production lines, and 1 1/2 inch diameter production tubing. The well currently has no rods in the wellhead and has a plug where the rods would go. The pumping unit looks to be off center with the wellhead and has no bridal currently on it. There appears to be a hook-up for house gas but the valve has been closed and line cut off. The valves leading to the tank battery are currently closed. There are small bubbles coming up from the stuffing box on the wellhead. The tank battery is equipped with two (2) 100 barrel steel tanks, a horizontal separator with a chart box attached to it. The chart box has no current chart in it or a meter to record any production. One of the tanks has a 50 barrel knockout tank on top of it. The valves on the tanks are closed at the time of inspection. The tank battery is very overgrown and looks to have not been maintained in some time. Near the tank battery there are some pieces of what appear to be well equipment near it including some 2 inch diameter black plastic production lines and a 4 1/2 inch diameter plug. This well is currently idle and not producing at the time of inspection. Poorboy Exploration Inc shall submit a written plugging schedule to the Brian Sefciks office at 5880 Memory Rd Zanesville Ohio, 43701-9553 within 30 days and plug the well within 180 days. Poorboy Exploration Inc will be notified if the plugging schedule is approved or denied.
1481901182 2016-12-15 Production Wells No Violations At 12:05 pm I missed a telephone call from Dave Bonar, owner of Poorboy Operating, at 1:54 pm I returned the call to Mr. Bonar. Mr. Bonar wanted to discuss the violation against the well. He stated that there was no way he could afford to plug the well in the next 180 days. I asked Mr. Bonar to send his written plan to Inspector Sefcik and include a time frame that he could meet.
2054059693 2006-09-25 Production Wells No Violations Upon inspecting well site found well to be idle last production reported in 1993 well is equipped with pump jack tubing and rods and 2-100 bbl tanks and separator all valves on wellhead are shut no domestic hookups noted it appears that possibly at sometime well was being used for domestic gas but dwelling across road has since been removed.
