0117539260 | 2013-03-05 | Production Wells | Violations Noted | Upon inspection, as part of a landowner complaint, I found the wellhead with all valves shut and the steel production line running from the wellhead to the tank battery disconnected just inside the dike. There is another steel production line that runs just adjacent to the wellhead and also leads to the tank battery where it is disconnected inside the dike. The separator has been removed from inside the dike. The two orange and black plastic lines, which are attached to the tank and run towards the wellhead have been disconnected, so there is no way this well is producing. There is an access road just off of Twp Rd 76 where there are several meters. Two are labeled for Northwood and Artex. The 3rd isnt labeled but all three have charts showing production. There is a gas line with a flow meter that has an identification listed as the Clayton Allen 2A/#1571. I followed these lines and they run in the opposite direction from the actual Clayton Allen 2A. ID is incorrect and production has been consistently reported for a well that is not capable of producing. Operator needs to change the ID to the correct well. Operator must make well capable of production within 30 days; or plugging is required in 30 days. Failure to comply may result in forfeiture of bond. 3/8/2013: John Schneider with Stonebridge called and said he would get ID taken care of and he would look into idle well. |

1323266980 | 2011-12-06 | Production Wells | No Violations | Inspection this day due to complaint by landowner, Myron Allen, that well is not producing. He has not received royalties in years, believes that the operator is dumping brine and that the identification signs and meter are on the wrong sales line (believes they are for the D. Morris #7). My inspection revealed the pumping unit has been removed and the well is equipped with rods and tubing. There is no identification posted at the well or the tank which is located on a small hill beside the well. There is a large tree growing through the ladder on the tank. Down along the road I found a 2 line with a small flowmeter registering zero and labeled as the Allen well. This line is coming from the north, so it possibly could be for the D. Morris well. The Allen well is to the south and the valves are all shut. There are numerous plastic lines in area and cant tell where any of them are going or if they are in use. Annual production is being turned in for Allen well but it is not producing now (well valves are shut) and all indications are that it has not produced in years. Inspection from 2008 reports that well is shut in due to sales line problems. DNR records show the operator reported 50 mcf for 2010, 33 mcf for 2009. I will not write a Notice of Violation until requested. Pictures taken. I found no evidence of brine dumping. There is a small plastic tank along road that belongs to Northwood, along with master meters for Northwood and Artex. There is no indication where gas from Stonebridge is being metered. |
1324039707 | 2011-12-14 | Production Wells | No Violations | I re-inspected this well after getting word that the Dominion pipeline is now back in operation. I checked the small flowmeter below well identified as the Clayton Allen meter and it was still registering zero gas. If it had been metering the Don Morris well, as landowner contends, there should have been some gas moving through it. All evidence still shows this well is idle and not producing. |
1365433254 | 2013-04-08 | Production Wells | No Violations | Compliance Action Follow-up 117539260: Landowner informed me that Stonebridge had put a pumping unit on the well and had not used a culvert crossing the stream at the access road. Upon inspection, I found the stream had been cut off leaving a pool of water in middle of access road. They had not corrected the stream flow. I found the area completely cleaned up and an ID posted at the tank battery. There was a pumping unit hooked up to the well with new line valves. The tank was equiped with new valves and bull plugs. The 2 tubing was shut in but the 4.5 valve was open on one side which ran to tank and the other side was hooked to a new 2 line which ran back down to access road where it is tied into transmission line. It was shut in at the meter location as well as the access road. The meter has no chart. Rob Nutter arrived and said he would open the stream back up and was going to put a chart on the meter. He was going up to inspect the work and will call me once well is in production. |
1365775148 | 2013-04-12 | Administrative Inspection | No Violations | Compliance Action Follow-up 117539260: I called Rob Nutter with Stonebridge and he stated he put 15 gallons of solvent down hole to help clean wellbore. I talked again with him this morning and he said they are going to bring a service rig out to pull out and replace the downhole pump. He believes it isnt working properly however they have recovered some oil and brine and have about 18 pounds of pressure. He will call me beginning of next week. 4/24/2013: I bumped into Rob in the field and he said a service rig was on location today fixing downhole pumpjack issues. He will call me when the well is back in production. |
1698465883 | 2013-06-11 | Production Wells | No Violations | Well is now in compliance. The well is currently producing oil and water from the formation. Rob Nutter stated that they need to relieve the formation of this fluid in order to increase the gas pressure. The meter at the wellhead is not currently pressurized for reporting gas production. The oil and water are moving from the wellhead to the tank in the dike. |

2002480167 | 2019-02-06 | Production Wells | Violations Noted | I was on location today to follow-up on a complaint by landowner Myron Allen about this well not being in production and the production lines on his property. At the time of my inspection I found the well to appear idle and not have been producing. The last production reported on this well was in 2013. The well owner/operator shall submit a written plugging schedule to (Justin Smith) at (5880 Memory Rd Zanesville Oh, 43701) within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
