He said he was having personal and heart issues at this moment and did not think he would be able to plug the well by February of this year. I called Supervisor Eric Conrad about the extension. He asked me to send all the information to him including the original inspection so he may pass it on to Deputy Chief Dave Clause. I received an e-mail today from Supervisor Conrad informing me that we will be giving Larry Lewis an extension on this well until August and to call Larry and let him know. I called Larry Lewis and informed him his extension was approved and he had until August to plug this well. |

At the time of inspection there were no 2 inch diameter production lines attached to the production valves and the valves on the well head were closed. The well head is not currently equipped with a pumping unit. Rods are currently sticking out of the well head. At the time of inspections the well is currently not capable of any production due to no pumping unit or production lines being equipped to the well and the valves on the well head are closed. The tank battery is to the left of the well and has 3 100 barrel tanks in a proper SPCC dike for containment. The well owner/operator shall submit a written plugging schedule to the Brian Sefciks office at 5880 Memory road, Zanesville OH, 43701 within 30 days and plug the well within 180 days. At this time the valves are still shut on the well but a new flow line has been ran to the tank pad. It looks like there is all new plumbing at the tank pad for the production but the valves are still shut making the well incapable of production. I tried to contact Mr. Lewis to see when he would have the well back in production but I did not reach him. I left a message for him to call me back about the well. After talking with Mr. Lewis he said he should have it completed in a couple of weeks and will contact me to look at it. |

I witnessed this day a well with rods, tubing - the pumping unit is gone and well is not in production, not tied to any lines. No identification, altho small one on the 3 100 bbl tanks inside a dike that does not meet SPCC requirements. I will send this inspection to Columbus to get the status changed to PR. The tanks are clean and it appears owner is working on improving the tank area. |
