0083277509 2014-12-01 Production Wells No Violations Bob Masters has installed some new lines and new drips at the tank battery location. They are trying to cleanup the well and the lines in order to get the gas into Artexs transmission line. The well and tank battery have recently been cleared of vegetation.
0253848123 2017-11-03 Production Wells Violations Noted I was on location today to do a field inspection on this well. The well appears to be in production at this time but I was unable to locate any identification for this well. The well by law must have legible identification located at either the well or the production tank. If there is more then one well producing into the same production tank the identification is required to be on both the well and the production tank. The identification must have the following, owner, well name, well number, permit number, township, county and emergency contact number. You will have 10 days to correct the violation and get the identification on either the well or the tank pad. I was able to contact Hocking Hills Energy to let them know about the violation today. Doug Stated he would get the violation resolved in the 10 days given.
0567573004 2013-07-11 Production Wells No Violations Dave Ball and I met Paul Withers at the well location to discuss the leaking casing issues. He had used a backhoe to dig out around the casing several feet deep. Gas was bubbling out of the side where there was a casing clamp. It was determined that a service rig would have to come into location and they would have to get a wellhead on the casing and pull the tubing in order to see how deep the well was and the condition of the packer. RBDMS records only show that the 7 was mudded with 10 sacks of fireclay at 1678. The proposed depth was 5000 and there is a history of H2S with this well. Paul said Lee Atha will call me to let me know when they plan on moving to the location..

1372874385 2013-07-03 Production Wells Violations Noted Upon inspection, I found the wellhead to be leaking gas from the surface casing somewhere around ground level. There was an audible sound and the gas detector alarm went off for methane as I was standing near wellhead. There is an H2S smell around the wellhead as well as the tank battery up by the road. This tank battery has two 1500 gallon plastic tanks and 4 separators. There is a meter with no chart and everything appears to be shut in. There are no other wells listed in RBDMS in this area. There is another 1500 gallon tank and a small shed straight up from the wellhead by the road, and the shed has a drum full of fluid. Everything is completely overgrown in this area. There is no identification posted for this well. I spoke with Bob Atha and he stated they will have someone out there soon to take care of the issues. He said the well was in the process of being transferred over to someone with Cortland Energy. Operator must post proper id. Operator must fix casing leak and make well capable of production within 30 days; or plugging is required in 30 days.
1373976429 2013-07-16 Administrative Inspection No Violations I talked with Bob Atha and he and Lee Atha are trying to line up a service rig with the hopes of getting the issues resolved before the middle of August. They will call me in order to inform me when they will be on location with the service rig.
1375810331 2013-08-06 Production Wells No Violations Compliance action follow-up 1372874385: I met with Lee Atha and he had put plugs on the wellhead with a gauge. The wellhead pressured up and held 250 psi and there was no evidence of any leak in the casing. He is going to contour out around the wellhead and leave the excavated area exposed should there be any issues in the future. He is also going to send me all of the missing records for this well and the Van Horne #2 down the road. Cortland Energy is now the owner of the well.
