0048405773 2016-01-15 Administrative Inspection No Violations Rob Rap the land owner called me today asking if there was any progress on plugging the wells or heard anything from G-R. I informed him I did not and would be calling them in the next few weeks to see what they have done or submitted a plan. I asked him if he would like to meet with myself and a G-R representative out at the wells. He said if I do that to let him know so he can try to be in town for it. He also informed me as of Monday the line still had not been fixed and that someone from the county had spoken to him claiming it was not G-Rs line but a county line and they would fix it. I will be going out to take a look early next week and will make the appropriate calls when necessary.
0743385999 2016-05-20 Plug / Plug Back No Violations I was on location to witness Formation Cement Company set a Berea plug (1620-1220), a rip plug (739-539), a bottom of surface plug (330-150), and a surface plug (150-0). There was circulation on all plugs and some oil. They used a total of 130 sacks of Class A cement. After cementing the bottom hole plug the 1 1/2 tubing got stuck in the hole. We then used Appalachian to log the 1 1/2 tubing to see where we could get free pipe. We established that at 744 in the hole was where we would shoot both the tubing of and the 4 1/2 casing and would set our rip plug there. This well has been plugged and all violation have been resolved.

0878744928 2016-08-12 Final Restoration No Violations I got a call from Jordan Jones on 8/10 saying that he believed the 4 wells off of Center Township Road (23216, 23220, 23217) were ready for inspection for final restoration. The inspection revealed that there was some vegetation growing in but there were still some lines associated with the well nearby. I called Jordan to talk to him about them but got no answer so I left him a message. I am still waiting to hear back from him.
1593076688 2011-04-01 Production Wells No Violations Request from landowner as to status, owner of wells, how he could get some issues of erosion, haning electric lines addressed. He is not mineral owner. Well this day found with jack, capable of producing but production reports show all these Bruns wells below the standards for economic commercial production. No IDs at well, pumper says they are down on tank at road. Pumper also says they should have wells sold by 4/15 - was wanting to wait and have new owner address.
1768298726 2016-03-22 Production Wells No Violations Rob Rap called me over the weekend and was asking if I had been out to the wells any time or heard anything from G-R. I informed him I would visit is this week and let him know any updates. Upon inspection I found there to be no resolution to any of the issues.
2000715959 2015-12-22 Production Wells Violations Noted On my inspection today I was accompanied by Inspector Parkins. This well a 4 1/2 inch well head and 1 1/2 inch tubing, and the surface casing is currently buried. The production line is currently equipped with two 2 inch ball valves and both are currently open. No production has been reported for this well in 2 years. The polish rod is oxidized leaving Inspector Parkins and I to believe it has not been pumped in a significant amount of time. The well is currently equipped with a pump jack. The well has no ID on it. There is a pipe line leading from this well to the shared tank battery and the plastic lining has a tear in it and is currently leaking gas. The well owner/operator shall submit a written plugging schedule to the Brian Sefciks office at 5880 Memory road, Zanesville OH, 43701 within 30 days and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Brian Sefcik can be contacted via his cell phone at 740-255-2833. Called Jordan Jones to notify him that the well was idle and that the pipeline had a leak in it. He let me know that someone will be going out as soon as possible to repair the pipe.
2008789001 2016-11-02 Final Restoration No Violations This well was plugged and abandoned earlier this year. The inspection today revealed that this well has had all equipment removed, vegetation has been established, the pit has been closed and buried, and erosion has been controlled. This well has passed final restoration in accordance with ORC 1509.072(B).
