0602111990 2018-02-08 Production Wells No Violations I returned to this well today to do a follow-up inspection on it. The well is no longer in violation. The company has posted their identification for the well on the production tank and the well now appears to be capable of production.
0925123090 2013-04-08 Annular Disposal No Violations Well is not hooked up for annular disposal. The well is currently shut in however Artex is current on production.
1402052803 2017-10-05 Production Wells Violations Noted I was on location to do a field inspection of this well. During the inspection of this well I found the well is not in production and is incapable of production with all valves at the wellhead shut. There has been no production reported on this well since 2012. I had a .1 ppm reading of H2S in the air during my inspection. The well owner/operator shall submit a written plugging schedule to (Justin Smith) at (5880 Memory Road Zanesville, Ohio 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
