1082031439 2004-04-14 Production Wells Violations Noted Upon inspecting tank site found the following violations pollution/contamination and illegal brine disposal at site. Large kill area below tank site southwest side.

1178152753 2007-04-30 Annular Disposal No Violations Complaint called in to Ohio-EPA of gas leak on landowner property inspected site and found leak contacted company rep and he sent out local well tender to place repair clamp on line small amout of oil stain on vegetation suggested they clean up affected area ASAP they said they would check with supervisor to see if cleanup was needed,landowner Michael Decker also wants unused equiptment removed from site suggested that company would work on getting equiptment removed from site as weather permits.
