0627896796 2013-01-08 Production Wells No Violations Compliance Action Follow-up 1085200291:I did a follow up inspection and found the dike, road, and stream to be cleared of contamination and pollution. Sawdust and straw were spread out over the entire area. The dike and front part of access road were recently graveled by owners son Randy. He was out there with another hand cleaning up area and had a backhoe on site. Once they are finished with the job I will come back and do a final inspection of well and location to ensure the removal of all pollution and contamination. An extension was given in order to complete the job.
0915432571 2013-01-17 Production Wells No Violations Compliance Action Follow up #1358274191: The workers were out on location continuing witht the cleanup of contamination. I told them I would come back soon to reinspect. Well owner is in process of selling this well and some others to Robert Nutter.
1085200291 2013-01-03 Production Wells Violations Noted General inspection revealed pollution and contamination in and outside of diked area due to tank overflow. Remediation efforts were made to clean up spill but were unsuccessful. The contamination was found in adjacent stream and measured roughly about 30 in travel length. Wellhead also revealed pollution and contamination due to leaking stuffing box. I called the well owner, Randy Rainer, and he thought area had been cleaned up. He was told all contaminated soil needed to be removed as soon as possible. He said he would be out there to throw some diapers down and get a backhoe to clean out stream. He also said he would get a pump truck to location to clean out dike. I drove by a short while later and Randy was out there to start the cleanup process.

Small area oil was still present on upside of culvert and I had them throw down some diapers. I told them I would return on Thursday to inspect the location. The wellhead was free of contamination as well as the dike and road. The dike has been repaired and is in good condition.

1556930778 2013-01-03 Production Wells No Violations Accompanied Jake Glasscock on an inspection of pollution and contamination along the lease road. A 100 Barrel production tank had overflow and oil had run down the side of the tank. The oil filled the dike and spilled over across the lease road and down a drainage ditch roughly 40 feet. The stuffing box had also allowed a small amount of oil to be released at the well head itself and contaminated an area roughly 5x5. The production company was called and clean up began that afternoon.
