Mullins well service emptied tanks. Contaminated soil was excavated and placed on a black 6 mil PVC liner, mixed with Bio Sorb, covered and secured for onsite remediation. Top soil was replaced and the storage tanks unhooked and removed from the dike. Status: Source of the contamination eliminated and on ground contamination remediated, will continue to monitor.

2109430906 2013-04-22 Production Wells Violations Noted Inspected the Chapman-Warwick Unit #1 (#3438) as part of a standard productive well status check. The well is equipped with a gas powered pump jack and appears to have been idle for some time. No production has been reported to the State of Ohio since 2003. The storage is located off State Route 95 and consists of two tanks (#39548 & #39547). Oil is actively leaking from tank #39548 at the T on the lower three inch nipple. Oil has also leaked/leaking from rear lower two inch line and commingled with the active leak and saturating the ground in a 7 wide x 19 long spot. The well identification and emergency contact information is illegible on the storage. The oil contamination at the storage needs remediated as soon as possible. The well identification and emergency contact information needs legible displayed and the Chapman-Warwick #1 needs to be placed back into production or plugged.
