0238835845 2012-09-22 Completion Testing No Violations Received a call from Tom Roberts (Scion Energy Consultant) that the steel completion tank used for flow back had a failure. Lost was about 45 barrels of flow back which was a combination of spent completion acid (15% HCL) and water. The path was easily traced across the location as it was constrained to preexisting ruts and swales. We tested puddles from the tank to the road ditch using PH paper and the flow back did not enter the road ditch and stayed on location. Miller Roustabout used a dozer to scrape up the soil (two blades wide) and piled it up at the back of the location. Plans are to let it dry out and bury it after mixing it with lime, spread lime over the location and plant rye for the land owner Mr. Lanker. Mr. Lanker was on location and was pleased with the remediation plans.

A gas separator and tank have been set 80 east of the well in a pad lined with a 2 earthen dike. The well identification and emergency contacts are clearly marked on the tank. The t-topped flare stack is set 125 from the tanks directly east into the farmers field which has been cleared of its beans around the stack. Stopped by the Morrow County EMA and alerted them to the operation, talked to Mr. Jeff Sparks who was going to let Joe Edwards (Morrow County Office of Homeland Security) know of the operation to avoid unnecessary calls.
