0334746196 2019-04-10 Production Wells No Violations The inspection was conducted as a Compliance Notice follow-up to number 1554392374 and complaint number 6123. I met with Jason Blocker the Emergency response officer of the Division at the site. The material was taken out and hauled to Washington county. New gravel was brought in and somewhat spread out. There were some absorbent pads placed near the culvert at the entrance. The identification has not been posted at the tank. I called the contractor that performed the work Ethan Eddy and left a message.

I explained that it takes about 15-20 thousand to plug the well. He asks about venting the gas to produce the well. I stated that you will no longer be able to go through pop-off device and will have to flare all gas. Well makes 41 barrel of oil and 105 Barrel of Salt Water per year. On the average of 6 dollars per barrel of disposal cost to figure in the annual cost of ownership should be considered.

1554392374 2019-04-04 Production Wells Violations Noted The inspection today is conducted under a complaint number 6123. There is a flowline leak that measures 15 feet by 226 feet. There is a culvert and then 72 feet by 3 feet of contamination and then an unknown intermittent stream. The company contractor stated the issue was found by the landowner on 3-3-[1] 100-barrel steel tank, [1] 50-barrel steel tank and a vertical separator that is enclosed within an earthen impoundment. There is no identification at the tanks. The company has 10 days to install the identification.

1501:9-8-2: (A) A reporting person shall notify the division by means of a toll-free telephone number designated by the chief and posted on the divisions website or by electronic means designated by the chief and posted on the divisions website within thirty minutes after becoming aware of the occurrence of any of the following unless notification within that time is impracticable under the circumstances: Violation:

1555333626 2019-04-05 Production Wells No Violations The inspection was conducted as a Compliance Notice follow-up to number 1554392374 and complaint number 6123. I was called by Jason Blocker the Emergency response officer of the Division at the site. The oil- soaked material was being removed and replaced with gravel. There was another source and the large hole was filling up with crude. Jason Blocker was uncomfortable about the issue. There is a down stream issue with going through the Corp of Engineers property. I stopped and helped with the issue. There was an unknown second line buried and had trapped crude around it. I had extra absorbent pads to lay out in the affected area.
