0204669041 2016-04-14 Final Restoration Violations Noted The follow-up inspection is due to a Temporary Restraining Order issued in the Trumbull County Court of Common Pleas and the Chiefs Order #2015-535, Chiefs Order #2016-105. 
The well was plugged and the Final Restoration will not pass due to equipment on site of plugged well and the service rig is still on location. The material at the site contains gravel pile, Electrical box and conduit. The material needs removed and site needs smoothed out and seeded and mulched. The 2-210 steel tanks are still at the location and have oil lying at the bottom of the tanks on the outside on the ground. The tank site and the well site were supposed to remediated by 11-1-15 per Partial Consent Order filed in the Common Pleas Court in Muskingum County, Ohio and signed by Big Sky Energy. Remove all tanks,pipes and other equipment assosiated with the Permit Number 3896. Big Sky has 14 days from date of issue to remove all contaminated dirt around and haul to an approved landfill. The Inspector will be notified before the work is started. The manifest from the landfill will be given to the inspector for the State Records.

0616981378 2015-05-04 Production Wells No Violations There was a pumping unit on the well head . The 2production manifold is open on the rod annulus. There is a electric motor and the belts are on the pulleys. The belts are new with a B-136 as the part number. The identification is at the power box. The rod has moved up and down a few times which is notated by the marks on the Polished rod. There are 2-210 barrel steel tanks that are at the tank farm. There is a flow line that has been repaired and hooked up to the 1-210 barrel tank. This line goes from 2steel to 1.25black plastic. The line goes up and in the top of the 1-210 barrel tank. There at the tank the 3discharge is connected with the other 210 barrel tank and piped together at the bottom. The 3line then goes to the ground and down over the hill buried. The 3line comes out of the ground with a 3valve and a bull plug that has not been taken off for sometime. They are in a spill box. I then went up to the electric line and new meter at the power pole. The Meter is hooked up but no voltage has ran through it. I had called Guernsey Muskingum Electric to verify this. The meter has 99999 on it. The tag is 28231. The bar code is 94-442-602. The tag at the bottom of the meter is 0067778 GMECO. I took pictures.

1001674044 2014-01-16 Production Wells No Violations Accompanied Inspector Pat Shreve on Chiefs Order follow-up. Well is equipped with a pumping unit. No motor was present at the time of inspection. Electric service was present at the pumping unit. On our way back to our vehicles, Inspector Shreve and I met Mr. Roger Barr on a tractor. He had an electric motor loaded on the back of it that he was delivering to the well. The tank battery is shared with p#3897. There are two production tanks in the tank battery and a separator. There is evidence of where there was one more tank but it was removed and there was oily fluid on that part of the impoundment. There was also a roll of black plastic line on site.

1382013598 2013-10-10 Production Wells No Violations Well is still not capable and there has been no change. The tanks and lines are not hooked up. No production being recorded since 2011. Pumping unit has not moved. Pictures were taken on day of inspection.
1391518419 2014-01-16 Production Wells No Violations Well is idle well with pumping unit and the polished rod has oxidized from non use. The tubing and rods are in place but there has been no power connected for some time due to the electric wires have oxidized also. The brakes on the gear box have not moved due to corrosion. Pictures were taken on 1-16-14 and a rock that was dated has been placed on the fly wheel to denote non movement. Pictures were taken of this also. I had a witness on the day of inspection named Andy Thomas to verify findings. On the way back to the vehicle Roger Barr was bringing in electric motor on tractor. He left as we were leaving. The line that will be used for flowline is holding about 25 psi of pressure. North of api# 3897 there is a hole dug and the flow line has been cut. There were 3 pictures taken. The company was trying to feed plastic line inside of 2steel flowline and failed. Company has Stated that this well will be plugged due to hole in the casing. The steel common tanks are leaking and need replaced if they will be placed in use.

There is no ID on the well or the tank. Produce or plug. Failure to comply with this request may result in forfeiture of bond.
1445523444 2015-10-21 Production Wells No Violations Well is still setting idle with pumping unit. The polished rod looks like there is some movement but there is no movement on the electric meter. The meter numbers are set 99999. The meter number is Guernsey Muskingum Rural Electric 94-442-602. Per my conversation with the utility company at 1:18pm on 10-22-15 they stated that the Pole and meter are wired and hooked up but there has been no usage. The electric is filed under Lighting Energy. The cob webs are off of the flywheel on the pumping unit. The Soy beans have been cut from field. There is no change at the tank site. Pictures were taken.

1450874253 2015-11-18 Plug / Plug Back No Violations On location at 8:00 AM to witness plugging operations. Upon arrival there was no service rig on the well. The well had rods and tubing in it. The service rig arrived at 9:30 AM. Greg Henderson arrived during inspection. Mr. Henderson informed me that would start pulling rods but probably would not be done by the end of the day. I left the location at 10:30 AM.
1489676520 2017-03-15 Plug / Plug Back No Violations The purpose of the Inspection is a Follow-up to Partial Consent Order on a Preliminary Injunction filed in Muskingum County Court of Common Pleas under Judge Fleegle on the case # CH2014-0231 filed on 8-25-15 and Chiefs Order #2013---50 barrel steel tank. The restoration and removal of all equipment at the tank site has not been completed on date 3-15-17. The contaminated soils need to be hauled away to an approved landfill and all manifests need to be provided for Inspector Shreve. Inspector Shreve will be present for the site clean-up.

1511751752 2017-08-02 Final Restoration No Violations On the day of my inspection, Brent Bear and I went in to the Powelson Shared tank site owned by Big Sky Energy INC. This inspection is a follow-up to Chiefs Order 2013-40 and Follow-up to Partial Consent Order on a Preliminary Injunction filed in Muskingum County Court of Common Pleas under Judge Fleegle on the case # CH2014-0231 filed on 8-25-15. The tank site shows that we still have the two-tanks on site for the production. There is oil at the bottom of the tanks and oil on the ground in small puddles around the 300 barrel steel tank to the north. There is old oil laying on the ground around the outside of the 210 barrel steel tank to the south. The horizontal separator is north of the tanks with various piping lying on the site. To the east down over the hill is 1--300 barrel and the 1--2-17.

The well was plugged but the tanks and other equipment were left on the site. The final Restoration did not pass per inspector Patrick Shreve.
1616357393 2016-08-03 Final Restoration No Violations The purpose of the Inspection is a Follow-up to Partial Consent Order on a Preliminary Injunction filed in Muskingum County Court of Common Pleas under Judge Fleegle on the case # CH2014-0231 filed on 8-25-15 and a Civil Action Follow-up and a Temporary Restraining Order issued in Trumbull County Court of Common Pleas and Chiefs Order # 2015-535 and Chiefs Order # 2016-105. The day of inspection revealed the following on the well site: there was a pile of gravel left approximately 3-4 ton and the area was vegetated back sufficiently. The tank site has not changed. The 2-210 barrel steel tanks are on site and leaking around the bottom of the tanks causing oil to puddle around the tanks and piping. The horizontal separator is still on site along with various piping. Down over the hill is a 50 barrel steel tank at the corner of the property with piping still intact. Pictures were taken by Inspector Patrick Shreve on day of inspection.
1628616287 2015-09-16 Production Wells No Violations Inspection revealed that there was no rig over the well. The well has an identification on the electric box. The polished rod is oxidized from non movement. The tank site has a line hooked up from the flow line to the tanks. There are 2-210 barrel steel tanks and 1-70 barrel steel tank. The separator is not hooked up at all. The electric box has not been turned on or connected.
