Absorbent pads were installed and hay bales installed on top on sections of creek, which had contained oil. Absorbent booms were replaced as needed. The spill remains contained at this time. Instructed Mr. Dwayne Dugan to remove all hay, pads and booms from location as they become saturated.

1341926812 2012-07-02 Production Wells Violations Noted On 7/2/2012, the Division received a complaint alleging that an oil spill had impacted Black Run Creek. The complainant alleged that this spill was not new because, booms had previously been installed. Note: Prior to receiving notification of this oil spill, the complainant called me at 12:24 and alleged that a dam had been installed in creek for drilling operations (PN 8766), which has resulted in an insufficient quantity of water for his cattle. I explained ODNRs jurisdictional boundary and instructed him to contact Muskingum Co. Oil & Gas Inspector, Pat Shreve. An inspection conducted with the well owner, Mr. Dwayne Dugan, Derrick Petroleum, confirmed that an oil spill had impacted Black Run Creek and was contained with absorbent booms. The creek contained low flow at time of inspection with intermittent sections of weathered oil around its bends. Approximately 1000 of this creek was inspected of which the last 100 contained no impact. The absorbent booms were installed throughout creek to ensure containment and a few would require replacement. Note: The largest section of oil was approx 40 x 10. At time of inspection, there was no impact to aquatic life. Mr. Dugan stated that the source of spill was from a 1.5 plastic line, which had ruptured and released approx 20-25 gallons around 5/15/2012, on top of a concrete bridge and located along the lease road. Further stating that Permit Nos. 7753 and 8762 were tied into this line. The oil entered the creek from this ruptured line location (oil stained concrete) and also flowed downhill to an area of lower elevation. This area contained a U shaped impact approx 75 x 10-25 with seed and straw applied to the surface. The soils were excavated from this area and placed along the lease road. New plastic has since been installed inside 3 steel casing. Mr. Dugan stated that he had previously been informed that notification was not required for small spills. 1398897619 2012-07-17 Production Wells No Violations A follow-up inspection was conducted with inspectors, Pat Shreve, and Kip Thompson. During the inspection, it was noted that progress had been made with the removal of oil from the impacted Black Run Creek. However, additional cleanup would still be required for the stream, as oil sheens still existed. The materials that had been utilized to absorb oil such as, pads and staw were now removed and two (2) booms remain installed for containment. Also noted. the earthen dam has now been removed from the stream. Previous inspections had revealed no observed impact to aquatic life from this spill. The majority of aquatic life observed in this creek since the spill, have been a substantial amount of frogs, however. 1 dead frog was observed today along the edge of water with no visual of oil. High Range Quantabs were placed in the stream below and above this location and showed an acceptable value of 0.8 units, Cl-. The US EPA recommended aesthetic value of 2 units equals 250 ppm (mg/L) Cl-. Lastly, the stockpiled topsoil along the lease road still need to be removed the location.
