0572809324 2016-02-09 Production Wells No Violations The purpose of the inspection is due to Compliance Notice Follow-up. The service rig was gone and the well is capable of production. The well is equipped with a conventional beam pumping unit. There is a 5.5x2.375 tubing head. The rod annulus is opened and the tubing annulus is opened to a pressure gauge which has 40 psi on it. There is an electric motor that powers the unit. There is 1-110 barrel steel tank that is not leaking and has bull plugs on the 2and 3valves. There is a 30 barrel poly tank that will need pumped out and removed. There is a dike but no identification. I called company and he will pump out the tank and get the electric hooked up. He will place identification at the tank.

Bill stated he will use Randy Moran. The other partner Dean Moran has walked away. Bill will call and meet on the week of 2-27-17.
0961854021 2018-05-14 Plug / Plug Back No Violations The inspection is conducted as a follow-up to Compliance number 1438183007 and complaint number 5091. The location was dry and clear and the current temperature is at 76 degrees Fahrenheit. The welder is on site welding the 5.5 inch and 7 inch collars to the casing. Wright well service ran tools into the 5.5 inch casing and ran into a clay bridge at about 1805 feet deep. Wright started to hammer with the jars and make some hole. The clay bridge was eventually broke through and ran to bottom. The tools were pulled out of the well and the knife was inserted. The tools were ran into the 5.5 inch casing and started ripping after there was 40,000 lbs./ft. of pressure pulled on 5.5 inch casing. They started ripping at 2300 feet deep on the 5.5 inch casing. There was no telegraph on the casing so Wright well service came up 50 foot increments. There was no telegraph all the way up to within the 7 inch casing. Wright well service shut down and the decision was made to bring JET Wireline Services in to the well to run bond log. Jet would pick a free space on the 5.5 inch casing and plug out.

1344031768 2018-05-15 Plug / Plug Back No Violations The inspection is conducted as a follow-up to Compliance number 1438183007 and complaint number 5091. The location was dry and clear and the current temperature is at 68 degrees Fahrenheit. Surveyor Tom Smith of Division of Oil and Gas Resource Management stopped by to get coordinates on the well. Jet Wire Line services were entering the site to rig up on the well. Jet wireline ran a bond log and tagged bottom clay plug at 3020 feet.. The bond showed that from 2400-1860 is open behind 5.5 inch casing. 1750-814 is 100 % bonded pipe on 5.5 inch casing with probably cement but has not been recorded. From 800-530 feet is around 50 % bond on the 5.5 inch casing. From 250-300 feet there was an open spot behind the 5.5 inch casing. From 250-
1438183007 2015-07-29 Production Wells Violations Noted I met with the landowner on complaint # 5091. The well is idle and incapable of production. The well is equipped with a 11.25conductor and 8.625casing inside. Then there is 5.5casing with 2.375tubing inside that. The pumping unit and the rods have been removed. There is no production lines connected to the well at all. There has been no production filed since 2008. There is 1-110 barrel steel tank and 13 barrel polymer tank that is full of water and oil and is exiting a 2hole near the top of the tank. There is no dike or identification at the well or tank. The company with owner #882 has 180 days to plug said well. Failure to comply may result in further Enforcement Action. Pictures were taken on day of inspection.

1513024294 2017-12-11 Production Wells No Violations The purpose of my inspection is due to a Compliance Follow-[1] 110 barrel steel tank and [1]550 gallon plastic tank. The steel tank is leaking at the burner tube. The tanks are enclosed within an earthen impoundment. There is no identification posted at the well or at the tank. I called owner at 13:45 and left message to call so tank can be extracted out and removed. Bill Kagle did not respond back. There has been no production filed with the Division of Oil and Gas since 1996. The company has 5 days from date inspected to get the tank extracted and removed.
1513618513 2017-03-07 Production Wells No Violations The purpose of the inspection is due to a Compliance Notice Follow-up to number 1438183007 and complaint number 5091. I met with Bill Kagle on his well and I explained that he has to have a plan and send it to me at the Zanesville office. I will then have to get approval from Deputy Chief. The well will have to be plugged this summer. He needs to file for his plug permit and get started. He stated he will have a contractor from Kentucky to come in and plug the well.
1526402053 2018-05-11 Plug / Plug Back No Violations The inspection is conducted as a follow-up to Compliance number 1438183007 and complaint number 5091. The location was dry and clear and the current temperature is at 80 degrees Fahrenheit. The well is currently under plugging operations and Wright well service has the service rig rigged up over the well. They are still pulling tubing out of the well. There was split string of 1.5 inch tubing and 2.375 inch tubing pulled from the well. Wright rigged up tools and ran in the well for a good depth. The tools set down at 3278 feet. Wright rigged up clay mixing equipment and washed in the bottom clay plug of approximately 2.5 ton of clay or approximately 250 shovels. The clay that was delivered from Belden Brick yard is very course. There are some small pieces of coal within the clay that were pulled out along with some small chunks of clay. The clay is placed in well very wet and extra water is run to push clay down. Wright well service shut down for weekend.

The well is currently having tubing ran in with service rig. The company is going to put well in to production. I will stop by on later date and perform another follow-up inspection.
2116849469 2019-04-10 Final Restoration Violations Noted The inspection was a Final Restoration, but it did not pass on the date of 4-10-19. There were some pieces of black 1-inch plastic to the west of the site down over the hill and no vegetation established on the old site. I called Bill Kaggle of Valley Dozer service and he would take care of the issues. The company has 30 day to restore the land back and have vegetation established and remove the plastic 1- inch pipe.
