1476641218 2016-10-16 Production Wells No Violations On this day I received a call at 12:30 pm from Inspector Eric Conrad who indicated the Caldwell Fire Chief Aaron Reed contacted Him in regards to a flow line leaking. I responded and upon my arrival found that a nearby well believed to be associated with the flowline had been shut in by the Fire Chief. The Chief who resides in the area of the well witnessed the flow line rupture. My inspection found approximately 2 gallons of Crude oil substance dispersed on vegetation and in a stream bed where the flow line had ruptured. I utilized spill rags and one oil absorbent boom to contain the product. The last know producer was contacted at 740-732-7251 and indicated She was receiving monies for the production but was not the responsible party and referred me to a Greg Wells 419-516-1095 & 330-863-1139, with an attempt to contact Mr. Wells unsuccessful. Photographs were taken. The Divisions Duty Officer notified prior to my response. The well is unregistered and producing gas into a Knox Energy system that is located in close proximity to the well. The call came in from the Caldwell Fire Chief as a private 3rd party call. The well was also equipped with 2 production casing only at surface that was selling into a 2 steel sales line that at some point underground changed over to the 2 plastic sales line that had ruptured. No production tank or metering equipment was present. No well identification was present.
1477399770 2016-10-25 Production Wells No Violations Status check inspection follow up to a gas line leak that occurred on 10/16/2016. Upon inspection it was observed that the 2 line had been repaired and the affected area addressed. The well is shut in and the producer has been sent a registration packet.
1666540271 2012-06-08 Orphan No Violations Inspection found 2 tubing only present and the production valve open and well being produced commercially by landowner , Inspector will make contact with landowner and attempt to have them register the well in their name .
