0208041518 2018-12-05 Production Wells Violations Noted The purpose of my inspection was to determine if pollution and contamination that was witnessed on 10/24/2018 had been remediated. On the day of my inspection I witnessed production equipment included a 210-barrel production fluid storage tank, horizontal separator, drip and 2-inch diameter meter run with a .250-inch diameter orifice. There was a small leak on the dump valve of the separator and on the inlet side of the separator. Production equipment was contained within an earthen dike. The well was equipped with 4.5-inch diameter production casing, 1.5-inch diameter tubing with rods, and a pumping unit with electric motor. All valves were open at the time of my inspection. There was approximately 40 square feet of pollution and contamination around the well head and it appeared that the 8.625 inch diameter surface casing well head was leaking. There were no signs of remediation. There was identification on the production equipment, but the well was not identified. Production had been reported for 2017.

There was identification on the production equipment, but the well was not identified, and production had been reported for 2017. At the time of my inspection it appeared that Northwood Energy had attempted to remediate the pollution and contamination and the dike in front of the well head had been repaired.
