Well appeared capable of production,no violations this inspection

1424126746 2019-04-12 Production Wells No Violations The purpose of my inspection was to follow up on previously issued compliance notice. Production equipment for the well included a 210-barrel steel production fluid tank, horizontal separator and 2-inch meter run with a .250-inch orifice, and drip. Also, in the production area was an additional horizontal separator, drip and a small natural gas compressing unit, there were oil filters and trash near the compressing unit. The additional horizontal separator was acting as a fluid drop out before the compressor. The production area also included 2 concrete brine vault tanks to the north of the steel production fluid storage tank, the brine tanks did not appear to be in use anymore but still contained fluid, all production equipment was enclosed within an earthen dike. The dike did not appear to be capable of holding fluid and is located near a steam. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, and 1.5-inch diameter tubing with a gate valve that was open, the flow line valve was slightly open, the flow line appeared to have been recently repaired and the well appeared to be producing. Identification was present and production was reported for 2017. I contacted Carl West operator for Northwood Energy and informed him of the issues that needed to be addressed, and updated landowner of the situation.

1541539082 2018-11-02 Production Wells Violations Noted The purpose of my inspection was to follow up on complaint number 6004. I received a phone call from Landowner Linda Slater who informed me that the well on her property had a gas leak between the well and production equipment. Linda explained that there were multiple leaks throughout the flow line, and the line had been leaking for a couple of months. I contacted ODNR Regional Supervisor Brent Bear and informed him of the complaint, Brent Informed me that he would accompany me during my investigation of the complaint. I spoke with Northwood Energy Corps Cory McCudden who informed me that they were aware of the problem, and had done multiple remediations on the flow line and the well was supposed to be shut in. Northwood Energy Corp sent out their operator for the well George Flowers who informed me that he had shut the well in several months ago and it was not supposed to be producing. Production equipment for the well included a 210-barrel steel production fluid tank, horizontal separator and 2-inch meter run with a .250-inch orifice, and drip. Also, in the production area was an additional horizontal separator, drip and a small natural gas compressing unit. The additional horizontal separator was acting as a fluid drop out before the compressor. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, and 1.5-inch diameter tubing with a gate valve that was open. The casing valve was closed, and the 2-inch diameter flow line valve was open at the time of my inspection. George Flowers the contract pumper for the well shut the 2-inch diameter flow line valve. Brent Bear and I remained on location to confirm the gas leak had stopped. Photographs were taken of the affected area and equipment. I informed Northwood Energy Corp Cory McCudden that the well can not be put back in production until the line is repaired and they needed to get the line repaired within 30 days.
1597551050 2018-08-06 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing with a tubing head, 1.5-inch diameter tubing, and a plunger lift system. The valve attached to the 4.5-inch diameter production casing was in the closed position. The Demco valve on the tubing and the ball valve on the sales line were in the open position. The production equipment consisted of one 210-barrel tank, one horizontal separator, and a sales meter. The sales meter had a 2-inch diameter sales line, a .25-inch orifice plate, and a chart recorder. The production equipment was located within an earthen dike that was shared with a small compressor and a horizontal separator labeled Jadco Energy. The compressor was in operation during inspection. Northwood Energy has submitted reports of gas production for the year 2017. Identification was located on the 210-barrel tank.
